Title: | Cognição social na doença de Parkinson e na paralisia supranuclear progressiva: uma investigação neuropsicológica |

Abstract: | O diagnóstico diferencial entre a Doença de Parkinson (DP) e Paralisia Supranuclear Progressiva (PSP) é um desafio. Há evidências de que pacientes com DP e PSP têm déficits de teoria da mente (TdM) e de reconhecimento de emoções faciais, duas habilidades de cognição social (CS). Apesar de muitos trabalhos já terem investigado aspectos cognitivo-comportamentais tanto na DP quanto na PSP, a literatura é escassa quanto à comparação direta entre pacientes com DP e PSP em medidas de CS. Por isso, estudamos dezoito pacientes com DP, vinte pacientes com PSP e vinte e três indivíduos controles saudáveis. Os três grupos foram pareados em relação à idade, sexo e escolaridade, e nenhum deles tinha história de outros transtornos neurológicos ou psiquiátricos. Cada participante foi submetido a uma avaliação neurológica e a um exame neuropsicológico, incluindo a bateria de CS “Mini-SEA”, a qual avalia reconhecimento de emoções faciais e TdM (teste de faux-pas). Pacientes com PSP tiveram desempenho pior do que os controles nos testes de TdM e de reconhecimento de emoções. Os pacientes com DP diferiram dos controles apenas no teste de TdM. Não houve diferença no teste de Reconhecimento de Emoções Faciais entre os grupos DP e PSP. Os pacientes com PSP tiveram pior desempenho do que os pacientes com DP no teste faux-pas. Essas diferenças se mantiveram mesmo controlando-se para o grau de disfunção executiva. Os presentes resultados corroboram a possibilidade de que a avaliação de CS contribui no diagnóstico diferencial entre DP e PSP. |
Abstract: | The differential diagnosis between Parkinson's Disease (PD) and Progressive Supranuclear Palsy (PSP) is a clinical challenge. There is evidence that patients with PD and PSP have deficits in theory of mind (ToM) and facial emotion recognition, two social cognition (SC) skills. Although many studies have already investigated cognitive-behavioral aspects in both PD and PSP, the literature is scarce regarding the direct comparison between patients with PD and PSP in measures of SC. Therefore, we studied eighteen patients with PD, twenty patients with PSP and twenty-three healthy controls. The three groups were matched for age, sex and education, and none of them had a history of other neurological or psychiatric disorders. Each participant was submitted to a neurological assessment and to a neuropsychological examination, including the “Mini-SEA” battery for SC, which assesses recognition of facial emotions and TdM (faux-pas test). PSP patients performed worse than controls in the ToM and emotion recognition tests. PD patients differed from controls only in the TdM test. There was no difference in the Facial Emotion Recognition test between the DP and PSP groups. Patients with PSP performed worse than patients with PD in the faux-pas test. These differences were maintained even after controlling for the degree of executive dysfunction. The present results suggests that the assessment of SC contributes to the differential diagnosis between PD and PSP. |
