Abstract: | This paper had the main objective of proposing an intervention for the solution of a priority problem for the Red Team at the Vale do Sol Primary Health Care Unit Ipatinga, Minas Gerais, namely the low adherence of patients with hypertension and diabetes enrolled in a specific dabatase (HIPERDIA) to the treatment plan. Considering the 558 patients with hypertension registered in HIPERDIA, only 456 (81.72%) followed the proposed pharmacological treatment and among the 152 diabetic patients registered in the same database, only 124 (81.57%) adhered. The elaboration of the intervention proposal followed three stages: context diagnosis, review of the literature and elaboration of the Plan of Action, based on the Strategical Simplified Context Planning and including a component of follow-up/monitoring of the adherence of the patients. A more effective follow-up of patients with hypertension and diabetes in the area covered by the Red Team is expected after the execution of the proposed Plan of Action. |
