Abstract: | Nos últimos anos ocorreu um crescimento exponencial no número de transplantes realizados mundialmente. Seu grande limitador é a rejeição, que pode ser mediada por reação celular e/ou humoral. O uso de drogas imunossupressoras tem por objetivo o controle deste fator. Desta forma, os pacientes submetidos a transplantes são um dosprincipais grupos de imunossuprimidos, e estão sujeitos à infecção por espécies de Aspergillus. Estas infecções acometem de 2 a 26% de pacientes submetidos ao transplante de medula óssea e entre um e 15 % dos pacientes que receberam algum órgão sólido. A taxa de mortalidade entre os transplantados com aspergilose invasivavaria entre 74 a 92%, e é estimado que 9,3 a 16,9% de todas as mortes em pacientes no primeiro ano após o transplante é atribuída à aspergilose invasiva. Devido à relevância epidemiologia da aspergilose em pacientes transplantados e principalmente à alta taxa demortalidade desta doença, e crescente aumento de Aspergillus spp. resistentes às medicações disponíveis, faz-se necessário um estudo recente sobre a fisiopatologia da aspergilose, visando destacar os pontos cruciais para o desenvolvimento de ações que previnam o surgimento de novos casos e apontar os melhores métodos diagnósticos e formas de tratamento. |
Abstract: | In the last years there is been an exponential growth in the number of transplant. The great problem is the rejection, which can occur by cellular and/or humoral response. The use of immunosuppressive drugs aims to control this matter. Thereby, the patients who have been through transplant are a significant group of immunocompromised people and they are more susceptible to be infected by species of Aspergillus. Infections by Aspergillus occur in 2 to 26% of patients thathave been through bone marrow transplant and in 1 to 15% of patients that received any solid organ. The mortality rate in transplanted people caused by invasive aspergillosis varies between 74 and 92% and it is estimated that 9.3 to 16.9% of all deaths in the first year after the transplant are due to invasive aspergillosis. Due to the epidemiological relevance of aspergillosis in transplant patients and mostly to thehigh mortality rate of this disease and also the increasing number of Aspergillus that are resistant to the available drugs, it is necessary an updated research about the aspergillosis pathophysiology in order to highlight the major points to develop actions that would avoid the emergence of new cases and to point out the best diagnosticmethods and treatments |
