Title: Efeito da valvuloplastia mitral percutânea sobre a complacência do átrio esquerdo em pacientes com estenose mitral reumática

Abstract: Introdução: A valvoplastia mitral percutânea por balão (VMP) é o tratamento de escolha para a maioria dos casos de estenose mitral (EM) grave. Seus efeitos em diversos marcadores prognósticos da EM como a área valvar mitral e o gradiente transmitral são bem conhecidos. Recentemente, surgiu nova evidência de que a complacência das câmaras cardíacas esquerdas pode ser um novo parâmetro prognóstico importante na EM grave. Os efeitos da VMP nesses parâmetros não está definido. O objetivo desse estudo foi determinar a resposta da complacência atrial e complacência atrioventricular à VMP bem como definir se essa mudança teria algum efeito na melhora funcional dos pacientes. Métodos: Noventa e quatro pacientes com EM grave e indicação para VMP no Hospital das Clínicas da UFMG foram incluídos no estudo. Todos os parâmetros ecocardiográficos e hemodinâmicos foram registrados antes e após a dilatação valvar. A mudança nas complacências foi correlacionada à mudança da capacidade funcional. Resultados: A idade foi de 43 ± 13 anos com 86% do sexo feminino. A maioria dos pacientes estava em classe funcional da NYHA II e III (78%) e apenas 7 pacientes (7%) estavam em classe IV à entrada no estudo. O procedimento resultou em aumento significativo da área valvar mitral com redução dos gradientes transvalvares, redução da pressão no átrio esquerdo e na pressão arterial pulmonar. Na população geral, a complacência atrial esquerda melhorou de 6,79 ml/mmHg [4,57- 9,23] para 12,69 ml/mmHg [7,34-26,88] (p<0,001) e a complacência atrioventricular melhorou de 2,82 ml/mmHg [2,02-4,08] para 3,21 ml/mmHg [2,33-4,66] (p<0,001). A complacência ventricular esquerda não alterou após a VMP. Esses efeitos foram observados apenas nos casos em que a VMP foi bem sucedida. A melhora da complacência atrial se correlacionou com a melhora da capacidade funcional dos pacientes (OR ajustado 0.67; IC 95% 0.45 - 0.97; p=0,036), independente de outros parâmetros prognósticos clássicos como área valvar mitral, pressão sistólica na artéria pulmonar, gradiente transmitral ou regurgitação mitral. Conclusões: A VMP resultou em aumento das complacências atrial e atrioventricular esquerdas e isso se correlacionou com a melhora da classe funcional dos pacientes durante o seguimento de 6 meses, independente da melhora em outros parâmetros prognósticos.
Abstract: Introduction: Percutaneous mitral valvuloplasy is the standard treatment option for severe mitral stenosis (MS). Its effects on several prognostic markers of MS such as mitral valve area and transmitral pressure gradient are well known. Recently there has been new evidence about left chambers compliance as an important parameter for prognosis is MS. The effects of the PMV on these parameters are not estabilished. The objective of this study was to determine the response of the left atrial and left atrioventricular compliance to the PMV as well as to define if these changes have any effect on the patients functional capacity. Methods: Ninety-four patients with severe MS and indication for PMV in the Hospital das Clínicas da UFMG were enrolled in the study. All the echocardiographic and hemodynamic parameters were recorded before and after the valvular dilatation. The change in left chambers compliance was correlated with the improvement in functional status. Results: The procedure improved significantly the mitral valve area and the transmitral gradient pressure and we also observed significant reduction of the left atrium pressure and the pulmonary artery pressure. In the overall population, the left atrial and the left atrioventricular compliance improved significantly from 6.79 [4.57- 9.23] to 12.69 [7.34-26.88] (p<0.001) and 2.82 [2.02-4.08] to 3.21 [2.33-4.66] (p<0.001). The left ventricular compliance did not change significantly. These effects were observed only in the patient where the procedure was successful. The improvement in the left atrial compliance correlated with the improvement in the functional capacity (adjusted odds ratio [OR] 0.67; 95% confidence interval [CI95%] 0.45 to 0.97; p=0.036), after adjusting for immediate procedural results, including postprocedural mitral valve area, systolic pulmonary artery pressure, mean transvalvular gradient and mitral regurgitation. Conclusions: The PMV improved significantly the left atrial and left atrioventricular compliance and this was correlated to the improvement of the symptoms in a 6 month follow up. These effects were regardless of the effects on other parameters of the severity of MS.
