Title: Em busca da integridade no SUS: conhecendo a integração da atenção básica à rede assistencial por meio da teoria de resposta ao item

Abstract: INTRODUCAO: A Politica Nacional de Atencao Basica (2011) define a Saude da Familia como estrategia para a consolidacao da atencao basica e as Redes de Atencao como referencia para organizacao de um cuidado integral. Evidencias sugerem serem fundamentais os dispositivos de regionalizacao, apoio matricial, comunicacao, informacao, regulacao e gestao compartilhada para o ordenamento da rede. Contudo, problemas relacionados a garantia de acesso aos demais niveis de complexidade do sistema configuram-se como importante desafio para a consolidacao do SUS. OBJETIVO: Avaliar a integracao da Atencao Basica a rede assistencial do SUS, utilizando os dados do PMAQ-AB, Brasil, 2012. METODO: Estudo transversal, com base nos dados de 17.202 equipes de saude da familia que responderam ao questionario de avaliacao externa do primeiro ciclo do Programa de Melhoria do Acesso e da Qualidade de Atencao Basica/PMAQ-AB. Para medir a habilidade de integracao das equipesa rede assistencial foi utilizada a tecnica de Teoria de Resposta ao Item por meio do modelo de resposta gradual de Samejima (GRM). Para estimativa do nivel de integracao foi empregada escala de habilidade (-3 a 3) as perguntas selecionadas no modulo II (Entrevista com Profissional da Equipe/AB). Posteriormente, definidos os intervalos para classificacao das Equipes: baixa; media e alta integracao. Os escores foram interpretados por meio da tecnica de visualizacao da nuvem de palavras, que propiciou obter qual a categoria de resposta do item mais frequente nos diferentes niveis de integracao definidos no estudo. Assim, foi possivel verificar impacto desses sobre o desempenho das equipes e identificar o comportamento das mesmas. RESULTADO: Dos itens utilizados no estudo, doze apresentavam otima discriminacao entre as equipes do PMAQ, pois carregam maior quantidade de informacao sobre a integracao das mesmas a rede. Desses, destacam-se osreferentes as acoes de apoio matricial: consultas medicas; discussao de caso; acao clinica compartilhada; construcao conjunta de projetos terapeuticos; atividade de educacao permanente; intervencoes no territorio e visita com profissionais de AB que tem melhor performance no nivel de media integracao e, apesar de nao exigirem alta habilidade das equipes, possuem os maiores valores na correlacao com a nota final (>500). Porem, o item discussao processo trabalho exige alta habilidade das equipes. Alem disso, os itens: Possui canal de comunicacao/Recebe Apoio NASF/Sempre; as vezescontato especialistas com AB/Frequencia apoio semanal, quinzenal e mensal/Possui lista contato com AB/Recebe Apoio CAPS/Possui de 1 a 2 centrais de marcacao/As vezes contato AB com especialista/Existe de 1 a 3 fluxos de comunicacao/Possui 3 centrais de marcacao/Semprecontato AB com especialista e Existe > 4 fluxos de comunicacao tem sua melhor categoria de resposta localizadas no nivel de alta integracao, quando observamos a tecnica devisualizacao da nuvem de palavras. CONCLUSAO: A realizacao das acoes de apoio matricial avaliadas no PMAQ-AB melhora o desempenho das equipes de saude da familia. Da mesma forma, porem em menor grau, os itens referentes a comunicacao reforcam esse entendimento. Contudo, apesar dos avancos conquistados na politica de saude do pais, a frequencia em torno de (50%) do pior cenario para itens do estudo, evidencia a necessidade de qualificacao da integracao entre as acoes de atencao especializada e atencao basica para consolidacao de uma Atencao Primaria a Saude abrangente
Abstract: INTRODUCTION: The National Policy of Basic Attention (2011) define Family Health as a strategy for basic attention consolidation and the Attention Nets as reference to the organization of an integral care. Evidences suggests that are of vital importance the actions ofregionalization, matricial care, communication, information, regulation and shared management for the network planning. However, problems related to the difficulty in access to the other levels of complexity in the system are a great challenge to the consolidation of SUS. OBJECTIVE: Evaluate the integration of the Basic Attention to the SUS attentionnetwork, using data from PMAQ-AB, Brazil, 2012. METHODS: Transversal study, based on the data from 17.202 family Health teams that answered to the external evaluation from the Program for Access and Quality Improvement in Primary Care/PMAQ-AB. To measure the teams' integration abilities to the assistencial network the Item Response Theory was used through the gradual response model Samejima (GRM). To estimate the integration level was used the abilities scale (-3 to 3) to the questions selected on module II (Interview with a team professional). Posteriorly, defined the gaps to classify the teams: low, medium and hight integration. The score were interpreted using the word cloud visualization technique that allowed us to get which one is the response category of the most common item in different levels of integration defined in the study . Then, was possible to verify their impact on theteams' performance and identify their behaviour. RESULTS: From the items used on the study, 12 had great discrimination betwin the teams on PMAQ, because they carry a greater level of information about their integration to the network. From them, are highlighted the ones refered to matricial support actions: medical appointment; case discussions; shared clinical actions; joined construction of therapeutic projects; permanent education activities; territorial interventions and visits with AB professionals that had their best performance in the level classified as medium integration and, despite hight abilitie is not demanded from theteams, have the highter values correlated with the final grade (>500). However, the item "discussion process work" requires high ability of the teams. Besides that, the items: Has communication channel/Has NASF support/Always - sometimes specialists contact AB/Frequency of weekly, biweekly and monthly support/Has contact list with AB/Receives CAPS support/Has 1 or 2 marking centrals/Sometimes - contact AB with specialists/There are from 1 to 3 communication fluxes/Has 3 marking centrals/Always - contact AB with specialists and There are and There are > 4 communication fluxes" has its best response category localized on the hight integration level, when we use the word cloud visualizationtechnique. CONCLUSION: The realization of the matricial support actions evaluated on PMAQ-AB improved the performance of the family health teams. In the same way, but in a smaller degree, the items referred to communication reinforce this understanding. However, despite the achievements in the country's health policy, the frequency around (50%) from the worse for the items of the study, highlights the necessity for qualification of the integration between the actions of Specialized Attention and basic attention to the consolidation of an embracing Health Primary Attention.
