Abstract: | Há evidências crescentes de que o ambiente em que as pessoas vivem é um determinanteimportante dos comportamentos obesogênicos. O objetivo deste estudo foi analisar aassociação entre o contexto da vizinhança e o comportamento sedentário e o hábito alimentarnão saudável da população adulta de Belo Horizonte, Minas Gerais. Trata-se de um estudotransversal, usando dados do sistema de vigilância de fatores de risco e proteção para doençascrônicas por inquérito telefônico (Vigitel) realizado no período de 2008 a 2010, na cidade deBelo Horizonte, Minas Gerais. O hábito alimentar não saudável foi avaliado a partir de umescore de alimentação não saudável com base no consumo de carne com excesso de gordura,refrigerante e carne vermelha e o não consumo de frutas e hortaliças. Foi consideradocomportamento sedentário o hábito de assistir televisão por quatro ou mais horas por dia.Informações georreferenciadas do ambiente físico (locais públicos e privados para a prática deatividade física, densidade de estabelecimentos com venda de alimentos, densidadepopulacional, densidade residencial) e social (taxa de homicídio, renda média total e índice devulnerabilidade a saúde) das áreas de abrangência das unidades básicas de saúde (AAUBS)foram inseridas na base do Vigitel. As variáveis sociodemográficas, de estilo de vida e desaúde foram utilizadas como ajuste. Para a análise dos dados, foram utilizadas a técnica devarredura scan e a regressão logística multinível. Foram estudados 5.783 indivíduos,distribuídos em 148 AAUBS. A análise espacial identificou cluster significativo de altaprevalência de comportamento sedentário e hábito alimentar não saudável em Belo Horizonte,mesmo após ajuste por características sociodemográficas. Ao comparar as AAUBS dosclusters com as AAUBS não pertencentes aos clusters, observaram-se diferençassignificativas no ambiente físico e no social. Vericou-se que viver em AAUBS com altoíndice de vulnerabilidade à saúde e baixa renda aumentam a chance de apresentar hábitoalimentar não saudável. Em relação ao comportamento sedentário, observou-se que viver emAAUBS com alta taxa de homicídio aumenta a chance de o indivíduo apresentarcomportamento sedentário. Essas associações permaneceram significativas após ajuste pelascaracterísticas individuais. As evidências encontradas neste estudo mostraram que o ambientesocial podem influenciar os indivíduos a se envolverem em comportamentos não saudáveis edevem ser incorporados em futuras intervenções e estratégias de promoção da saúde e reduçãoda obesidade. |
Abstract: | There are increasing evidences that the environment in which people live is an importantdeterminant of obesogenic behaviors. The aim of this study is to analyze contextual factorsassociated with the unhealthy eating habits and sedentary behavior of the adult population ofBelo Horizonte, Minas Gerais. This is a cross-sectional study using data from the SurveillanceSystem for Risk Factors and Protection for Chronic Diseases by Telephone Inquiry (Vigitel)conducted in the years 2008 to 2010, in the city of Belo Horizonte, Minas Gerais. Theunhealthy eating habits were evaluated from an unhealthy eating score based on theconsumption of meat with excess fat, soda and red meat, and, the non - consumption of fruitsand vegetables. Sedentary behavior was considered when the individual reported habit ofwatching television four or more hours per day. Georeferenced information of physical(public and private places for physical activity, density of food-retail establishments,population density, and residential density) and social environment (homicide rate, total meanincome and health vulnerability index) of the covered area of the basic health units(CABHUs) were inserted in the base Vigitel. Sociodemographic, lifestyle and healthvariables were used as adjustment. For analysis of the data, the technique of scan scanningand multilevel logistic regression were used. A total of 5,783 individuals were studied. Thespatial analysis identified a significant cluster of high prevalence of sedentary behavior andunhealthy eating habits in Belo Horizonte, even after adjusting for sociodemographiccharacteristics. When comparing the areas inside and outside the clusters, we observedsignificant differences in the physical and social environment. It was observed that to live inCABHUs with high health vulnerability index and low income increase the unhealthy eatinghabits. Regarding sedentary behavior, it was verified that the high homicide rate increases thechance of the individual presenting sedentary behavior. These associations remainedsignificant after adjustment for individual characteristics. The evidence found in this studyshowed that the social environment may influence individuals to engage in unhealthybehaviors and should be incorporated into future interventions and strategies for healthpromotion and reduction of obesity. |
