Title: Aceitação dos pais/responsáveis em relação aos métodos de contenção utilizados em crianças e adolescentes, portadores de deficiência mental, durante o atendimento odontológico

Abstract: Este estudo analisou a aceitação dos pais/responsáveis em relação às técnicas de contenção, utilizadas em crianças e adolescentes com deficiência mental, durante o atendimento odontológico. Participaram 209 pais/responsáveis de crianças/adolescentes, com idade inferior a 15 anos e portadores de deficiência mental, da Associação de Pais e Amigos dos Excepcionais (APAE) de Belo Horizonte-MG e de Lagoa da Prata-MG. Após a assinatura do termo de consentimento, o participante respondeu uma entrevista. Em seguida, todos assistiram a um vídeo exibindo as técnicas de contenção física (ativa e passiva), sedação e anestesia geral. Cada participante marcou, em um diagrama, se aceitava ou não o uso de cada técnica, durante o atendimento da criança ou adolescente. Os dados foram analisados pelo teste X2 e razão das chances. Houve diferença estatisticamente significativa na aceitação dos participantes quanto às técnicas de contenção (p=0,000), sendo a anestesia geral aceita por apenas 23% dos pais/responsáveis. No entanto, quando consideradas apenas às técnicas de contenção ativa (51%), contenção passiva (56%) e sedação (59%) não houve diferença estatisticamente significante entre estas técnicas (p=0,282). Portanto, apresentam o mesmo índice de aceitação entre os participantes. Através da razão das chances, o participante com maior chance de aceitação da contenção ativa tem 36 anos de idade ou mais, é pai ou mãe da criança/adolescente, possui até média escolaridade e pertence a APAE-LP. Seu filho está na faixa etária de 10 a 14 anos e é de classe econômica menos favorecida. O pai/responsável com maior chance de aceitação da contenção passiva é aquele cuja criança está na faixa de nove meses a quatro anos e possui experiência com contenção física. Nenhuma das variáveis independentes analisadas determinou o perfil do participante com maior chance de aceitação da sedação. O participante com maior chance de aceitação da anestesia geral é aquele cuja criança/adolescente está na faixa etária de 10 a 14 anos, pertence a APAE-BH e possui experiência com anestesia geral. Diferentes fatores influenciaram a aceitação dos pais/responsáveis quanto à aceitação de cada uma das técnicas. Portanto, o profissional deve estabelecer sua maneira de abordagem considerando a individualidade de cada paciente e família.
Abstract: This study analyzed the parents' acceptance about the restraint techniques, used in children and teenagers with mental deficiency, during the dental treatment. The sample included 209 parents of mental deficiency children younger than 15 years old, from the Association of Parents and Friends of the Exceptional (APAE) from Belo Horizonte (BH) and Lagoa da Prata (LP), in Brazil. After the signature of the consent term, the participant were interviewed. Soon afterwards, all the parents watched a video showing the techniques of physical restraint (active and passive), sedation and general anesthesia. Each participant marked the acceptance or the disapproval of the use of each technique in a diagram, during the child's dental care. The data were analyzed by the X2 test and odds ratio. There was a significant statistical difference among concerning the restraint techniques (p=0,000). The general anesthesia was only accepted by 23% of the parents. However, it was observed that there werent a significant statistical difference between the active restraint (51%), passive restraint (56%) and sedation (59%) (p=0,282). Therefore, the active restraint, passive restraint and sedation are likewise accepted by the participants. Through the odds ratio, the participant with major chance of acceptance of the active restraint is 36 years of age or more, is the child's father or mother, has low/medium education, his sons age is from 10 to 14 years old, belongs to a less favored economical class and belongs to APAE-LP. The participant with major chance of acceptance of the passive restraint has a child from the age of nine months to four years and experience with physical restraint. None of the independent variables analyzed were able to determine the participants profile with major chance of acceptance of sedation. The participant with major chance of acceptance of the general anesthesia has a child from the age of 10 to 14 years, belongs to APAE-BH and has experience with general anesthesia. Different factors influenced the acceptance of the parents for each one of the techniques. Therefore, the professional should establish an approach way considering the individuality of each patient and family.
