Planos de cuidados continuados e prevenção das complicações da Hipertensão Arterial Sistêmica em uma Unidade Saúde da Família da zona rural

Abstract

Systemic Arterial Hypertension is a common chronic disease in the population. In rural areas, treatment, care and prevention of complications are difficult. Hypertensive patients in rural areas face difficulties in accessing information that is even greater than in urban areas. The objective of this work is to elaborate an intervention project seeking the continued care and prevention of complications of Systemic Arterial Hypertension in the attached population of the Health Team of the Vanguarda III Family, in the rural area of the municipality Rubelita, state of Minas Gerais. The literature review was carried out, aiming to subsidize the preparation of the intervention project. The Virtual Health Library was searched through the databases of the Scientific Electronic Library Online, articles and Conclusion Works, using key words to search for articles, blood pressure, hypertension, Family Health Strategy, zone rural. The articles were published in the period from 2001 to 2016, and the selection of the articles occurred from January to April 2018. After the review an intervention plan was drawn up, based on the local reality diagnosis, using the Strategic Planning method Situational simplified. The main proposals were to expand information on the disease, establish healthy living habits for the population and strengthen health education actions of professionals. It was concluded that the proposed intervention plan is feasible and necessary, aiming to act in the prevention of complications of Systemic Arterial Hypertension and in health promotion. Considering the reality of a Family Health Unit in the rural area, planning strategies and monitoring is key to meeting the challenges.
