Blefaroplastia Superior Estendida com Incisão Angulada: aprimoramento técnico

Abstract

Introduction: Extended upper blepharoplasty (EUB) allows for the removal of hanging eyelid skin. Despite its good functional and esthetic results, it has inconvenient side effects that include scarring in the lateral periorbital region, Asian eyelids, and downward eyebrow traction. Purpose: To describe the technical change to the angles in the EUB lines. Method: Retrospective study involving 30 patients who underwent modified EUB with surgical incision angle at the upper and lower projection of the lateral canthal commissure, considering the right and left eyelids separately, between January 2018 and May 2019. The patients were studied for periods of longer than six months after surgery, and each answered a questionnaire about the scars’s esthetic results. Palpebral fissure inclination (PFI) was measured in pre- and post-surgical photographs. Eyelid assessment before and after surgery was conducted by the Student’s t test for paired samples and generalized estimating equations. Results: Lateral scars varied between 20.2 mm and 47.3 mm, averaging 30.4 mm. There was a PFI increase in 30 eyelids, and a PFI reduction in 29 of them, with an average variation of 0º05’ (p = 0.736). Patient satisfaction levels were 98.3%, considering lateral scars to be excellent or very good. Conclusion: Extended and angled upper blepharoplasty resulted in a small variation of the palpebral fissure, including its reduction and nearly unnoticeable lateral scar, with high satisfaction.
