Title: Perfil etário dos gastos públicos com saúde: uma análise temporal para o Brasil, segundo PIB per capita dos estados, sexo e tipos de gastos

Abstract: Compreender a evolução do perfil etário dos gastos com saúde ao longo do tempo, bem como os fatores que podem influenciar essa distribuição, torna-se fundamental para projeções de gastos mais precisas. No Brasil, ainda não se investigou completamente nenhum desses pontos. A fim de ampliar o conhecimento acerca da distribuição dos gastos com saúde por idade no país, esta dissertação se propõe a analisar as diferenças entre as Unidades Federativas quanto ao perfil etário dos gastos públicos com serviços hospitalares e ambulatoriais, focando em particular a relação da forma das curvas com o PIB per capita. Além disso, são investigadas as mudanças nesses perfis ao longo do tempo. A análise compreende o período de 2002 a 2016, para os gastos hospitalares, e os anos 2009 e 2016, para os gastos ambulatoriais. Investiga-se a relação entre o PIB per capita e a razão entre o gasto per capita dos idosos (65+) e o do restante da população (0-64). Nota-se que, em 2002, a correlação entre essas variáveis é positiva. No entanto, essa relação não se mantém ao longo do período considerado. A análise temporal dos perfis mostra que, nas UFs mais ricas (pertencentes ao quartil mais elevado do PIB per capita), ocorre uma inflexão da curva para baixo nas idades mais avançadas. No restante do país, a mudança se dá no sentido contrário, de aumento da inclinação da curva. Através de modelos de regressão linear são comprovadas essas diferenças. Destaca-se que, nas UFs dos três primeiros quartis do PIB per capita, são estimadas taxas de crescimento do gasto per capita significativamente superiores nas idades mais velhas, o que não rejeita a ocorrência de steepening do perfil etário do gasto com saúde na maior parte do país. Esse resultado traz à tona a preocupação quanto à sustentabilidade financeira do SUS, uma vez que a pressão sobre os gastos poderá ser significativa se a tendência de aumento da inclinação da curva continuar. Por outro lado, as evidências das UFs mais ricas indicam outra trajetória possível para o perfil dos gastos por idade no longo prazo, frente a um crescimento do PIB per capita no país. A constatação de que o perfil etário do gasto com saúde difere entre as UFs e que ele não se manteve constante, nos últimos anos, é importante para que tais dimensões também sejam incorporadas em análises sobre a trajetória futura dos gastos públicos com saúde no Brasil.
Abstract: Understanding the long-term development of the age profile of healthcare expenditures, as well as the factors that can influence this distribution, becomes essential for more accurate health expenditure projections. In Brazil, none of these points has yet been fully investigated. To increase knowledge about the age distribution of healthcare expenditure in the country, this thesis analyzes the differences between the Federative Units regarding the age profile of public expenditure on hospital and outpatient services, focusing in particular on the role of GDP per capita. Also, changes of the profiles over time are investigated. The analysis covers the period from 2002 to 2016, for hospital expenses, and the years 2009 and 2016, for outpatient expenses. To assess the effect of GDP per capita on the shape of the curve, we investigate the relationship between GDP per capita and the ratio between the per capita expenditure of the elderly (65+) and that of the rest of the population (0-64). It is noted that, in 2002, the correlation between these variables is positive. However, this relationship is not the same over the period. The analysis of the age profiles over time shows that, in the richest UFs (belonging to the highest quartile of GDP per capita), there is a downward inflection of the curve at older ages. In the rest of the country, the movement is towards increasing the slope of the curve. These differences are proven through linear regression models. It is noteworthy that, in UFs belonging to the first three quartiles of GDP per capita, significantly higher expenditure growth rates in older ages are estimated, which does not reject steepening of the age profile of healthcare expenditure in most of the country. This result raises concerns about the financial sustainability of SUS since the pressure on expenditures could be significant if steepening continues. On the other hand, the evidence from the richest UFs indicates another possible trajectory for the age profile of healthcare expenditure in the long run, with the growth of GDP per capita in the country. The finding that the age profile of healthcare expenditure differs between UFs and that it has not remained constant in recent years is important so that these dimensions are also incorporated into analysis on the future trajectory of public healthcare expenditures in Brazil.
