Abstract: | A verticalização de molares inferiores é um procedimento exigido com muita frequência na prática ortodôntica e que apresenta certa complexidade quando não está indicada a extrusão do molar, o que acontece na grande maioria dos casos. Uma das dificuldades encontradas é estabelecer uma unidade de ancoragem estável e eficiente. Este estudo sugere uma nova alça de verticalização com intrusão, chamada de Alça de Correção Radicular Modificada (ACRM), que tem uma ativação individualizada, visando atingir as forças e momentos ideais para o movimento. Foi selecionado um modelo de gesso e uma tomografia de um paciente adulto jovem que tinha um segundo molar inclinado. Acessórios ortodônticos foram colados no modelo e as forças liberadas pelas alças foram avaliadas por meio de um dinamômetro. A tomografia foi usada para obtenção de modelos tridimensionais para posterior discretização e simulação através do Método de Elementos Finitos. Testou-se três alças: a Alça de Correção Radicular de Burstone (com ativação sugerida por Marcotte), a alça preconizada por Sander e Wichelhaus e a ACRM. A tendência de giro e intrusão do molar foi verificada em todas as alças. A ACRM apresentou, quando comparada à Alça de Sander, apenas 40% do pico de tensão máxima principal na unidade de ancoragem e 58% em relação à alça sugerida por Marcotte. A ativação da alça de verticalização levando em consideração a distância entre o centro de rotação do dente e a linha de ação de forças, como feito com a ACRM, pode apresentar menos efeitos colaterais na unidade de ancoragem, favorecendo sua aplicação clínica e dispensando o uso de mini implantes em alguns casos. |
Abstract: | The uprighting of lower molars is a frequently required procedure in orthodontic practice and presents a certain complexity when the molar’s extrusion is not indicated, which happens in most of the cases. A difficulty is to establish a stable and efficient anchorage unit. This study suggests a new uprighting spring called “Modified Root Correction Spring” (MRCS), which has an individualized activation to reach the ideal forces and moments for the uprighting with intrusion. A plaster model and a tomography of a young adult patient who had an inclined second molar was selected. Orthodontic accessories were bonded in the model for evaluation of the forces released by the springs using a dynamometer. The tomography was used to obtain a three-dimensional model for discretization and simulation using the Finite Element Method. Three springs were tested: the Burstone’s Modified Root Correction Spring (with activation indicated by Marcotte), the spring developed by Sander and Wichelhaus, and the MRCS. The tendency of molar uprighting and intrusion was verified in all the springs. The MRCS showed, when compared with the Sander’s Spring, only 40% of top maximum principal stress in the anchorage unit and 58% in relation to the Marcotte´s spring. The activation of the uprighting spring considering the distance between the tooth rotation center and the forces’ line, as done with the MRCS, may present fewer side effects in the anchorage unit, favoring its clinical application and making the use of mini implants not needed in some cases. |
