Abstract: | A carga oclusal e sua distribuição são considerados fatores que influenciam no sucesso e falha dos implantes a longo prazo. A decisão de esplintar ou não implantes adjacentes com o objetivo de reduzir o estresse e aumentar a área de resistência sempre foi fonte de controvérsia. Muitos foram e continuam sendo os investigadores que buscam esclarecer essa questão por meio de vários métodos, incluindo análises de elementos finitos, análises de modelo fotoelástico e investigações clínicas. O objetivo do estudo foi revisar a literatura determinar em quais situações clínicas há indicação para a esplintagem de coroas implantossuportadas em segmentos posteriores visando à longevidade / manutenção da osseointegração. A decisão de esplintar ou não a prótese sobre implante depende do tipo de conexão, do comprimento e diâmetro do implante, relação coroa/implante e oclusão, passividade protética, das características do paciente, qualidade óssea e da técnica cirúrgica. Alguns estudos mostram que a esplintagem entre implantes não apresenta qualquer efeito negativo em sua longevidade quando o assentamento da estrutura for passivo. No entanto, a não ferulização das coroas propicia um assentamento passivo mais efetivo e uma diminuição na complexidade da execução do trabalho protético. Sugere-se que próteses esplintadas geram distribuições de tensões mais uniformes, mas esses dados não são estatisticamente significativos. Enfim, não há consenso sobre qual padrão protético (esplintado ou não) é superior. Este estudo pretendeu, através de uma revisão de literatura, auxiliar nesse esclarecimento. |
Abstract: | Occlusal load and its distribution is considered to be one of the principal components that influences the success and failure on implant-supported restorations over time. The rationale of splinting in implant dentistry to minimize stress by increasing the resistance area over which the load is distributed is controversial. Many authors try to clarify this question through different tests and methods, like, finit element analysis, fotoelastic models analysis and clinical trails. The objective of this study was to review the literature to determine in which clinical situations there is indication for splinting implant-supported prostheses in later segments aiming at their longevity/maintenance of osseointegration. The decision to splint or not implantsupported prostheses depends on the type of connection, length and diameter of the implant, as well as crown/implant and the occlusion ratio, passivity, patient characteristics and the surgical technique. Implants splinted together does not have any negative effect on their longevity when the nesting structure is passive. Splinted prostheses generated more uniform strain distributions; however, the strain distribution data in many articles were not statistically different from what was seen for the nonsplinted prostheses. Anyway, there is no consensus on which standard prosthetic (splinting or not) is superior. This study was intended to, exactly, contribute to this clarification. |
