Title: | Avaliação da microestrutura macular, utlizando tomagrafia e coerência óptica de domínio spectral (SD-OCT) em pacientes com oclusões venosas retinianas |

Abstract: | RESUMO DO TRABALHO: OBJETIVO: Investigar o efeito da adesão vitreomacular (AVM) sobre o resultado do tratamento antiangiogênico nas oclusões venosas retinianas (OVR) MÉTODOS: Quarenta olhos de 40 pacientes foram incluídos nesse estudo. Avaliaram-se a acuidade visual com correção (AVCC), a espessura macular central (EMC), e a espessura subfoveal da coroide (ESC) em pacientes com OVR no início do estudo, 1 e 3 meses após o tratamento com agentes anti-fator de crescimento endotelial vascular (VEGF). Todos os pacientes foram estratificados por tomografia de coerência óptica de domínio espectral em dois grupos: AVM (+) e AVM (-), de acordo com a presença ou ausência de AVM, respectivamente, e a resposta ao tratamento foi avaliada. RESULTADOS: Vinte e quatro olhos (60%) foram incluídos no grupo AVM (-) e dezesseis olhos (40%) no grupo AVM (+). Em comparações pareadas de AVCC entre o baseline e cada visita de seguimento (1 e 3 meses), ambos os grupos apresentaram uma melhora estatisticamente significativa. Quando foi comparada a magnitude de melhora entre os grupos foi observado uma melhora mais acentudada no grupo AVM (+), sendo tal diferença estatisticamente significativa no mês 1. Para ambos os grupos, comparações pareadas de EMC mostraram diminuição estatisticamente significativa quando os dados obtidos nos meses 1 e 3 foram comparados com o valor basal, não sendo encontrada diferença entre os grupos. A espessura suboveolar da coroide (ESC) apresentou redução nos dois grupos, foi observado que no baseline a ESC era maior nos pacientes do grupo VMA (+). CONCLUSÕES: Neste trabalho pacientes do grupo AVM (+) que foram tratados com terapia anti VEGF devido a oclusões venosas retinianas (OVR) apresentaram uma resposta mais significativa que os pacientes do grupo AVM (-), sendo tal diferença estatisticamente significativa no mês 1. Pacientes de ambos os grupos apresentaram redução da espessura macular central (EMC) e da espessura subfovelar da coroide (ESC), não havendo diferença estatisticamente significativa, em relação a magnitude desta redução, entre os grupos AVM (+) e AVM (-). |
Abstract: | AIMS: To compare anti-vascular endothelial growth factor (VEGF) treatment outcomes for macular edema secondary to retinal vein occlusion (RVO) based on vitreomacular adhesion (VMA). METHODS: Forty eyes of forty patients were included in this study. Best-corrected visual acuity (BCVA), central retinal thickness (CRT) and subfoveal choroidal thickness were measured in patients with retinal vein occlusion (RVO) at baseline and after 1 and 3 months of anti-VEGF treatment. Eyes were stratified by spectral-domain OCT results into two groups, VMA (+) and VMA (-). RESULTS: Twenty-four eyes were included in the VMA(-) group (60%) and sixteen eyes were included in the VMA (+) group. Both groups showed a statistically significant improvement in BCVA from baseline to month 1 and 3. When comparing the two groups, there was a more significant improvement in the VMA(+) group, whose difference was substantial at month 1. Both groups showed reduction in CRT from baseline to month 1 and 3, there was, however, no statistically important difference between the two groups. A decrease in SCT was observed in both groups from baseline to month 1 and 3, although the VMA(+) group had higher SCT at baseline. CONCLUSIONS: In this study, eyes of the VMA(+) group treated with anti-VEGF for retinal vein occlusions (RVO) had a greater improvement than eyes of the VMA(-) group, this difference was statistically significant at month 1. Both groups had a reduction in CRT and SCT, and there were no statistically significant difference between the two groups. |
