Título: | Influência dos fatores biomecânicos da pelve e dos membros inferiores na dor patelofemoral |

Resumo: | Apesar da alta incidência e prevalência da síndrome dolorosa patelofemoral (SDPF) os seus fatores causais ainda não são totalmente conhecidos. A etiologia da dor patelofemoral vem sendo amplamente estudada e, várias teorias têm sido propostas na tentativa de melhor explicá-la. O objetivo deste estudo foi verificar a influência dos fatores biomecânicos da pelve e dos membros inferiores na dor patelofemoral, por meio de uma revisão da literatura. Foram realizadas buscas nas bases de dados Scielo, Lilacs, Pubmed e ISI Web of Knowledge. Entre os fatores associados à síndrome, citados na literatura, encontraram-se o aumento do ângulo Q, a tensão das estruturas laterais do joelho, a tensão das estruturas anteriores do quadril, o posicionamento inadequado da patela, a insuficiência do vasto medial oblíquo ou seu atraso na ativação e, a pronação excessiva. Os fatores que apresentaram maior correlação com a SDPF foram o desempenho de força muscular do quadril, assim como a tensão da banda íleotibial (BIT), a cinemática do retropé e o atraso na ativação do vasto medial obliquo (VMO) em relação ao vasto lateral (VL). Portanto, sugere-se que estes aspectos devem ser incluídos na avaliação fisioterápica para o tratamento de pacientes com SDPF. |
Abstract: | Despite the high incidence and prevalence of patellofemoral pain syndrome (PFPS) its causal factors are still not fully known. The etiology of patellofemoral pain has been widely studied and several theories have been proposed in an attempt to better explain it. The aim of this was to check the influence of biomechanical factors of the pelvis and lower limbs in patellofemoral pain, through a literature review. We searched the databases SciELO, Lilacs, PubMed and ISI Web of Knowledge. Among the factors associated with the syndrome, in literature, found an increased Q angle, the tension of the lateral structures of the knee, the tension of the anterior structures of the hip, the improper placement of the patella, the failure of the vastus medialis oblique or her delay in activation and excessive pronation. Factors that had a higher correlation with the performance of SDPF were hip muscle strength, and tension band ileotibial(BIT), the kinematics of the rearfoot and he delay in activation of the vastus medialis oblique (VMO) relative to vastus lateralis (VL). Therefore, it is suggested that these aspects should be included in physical therapy evaluation for the treatment of patients with PFPS. |
