Implantação do hiperdia e a necessidade de melhorias no manejo e controle dos hipertensos e diabéticos na unidade básica de saúde Lapinha - Lagoa Santa - MG

Abstract

This intervention plan aimed to implement the program in the health strategy Hiperdia family Lapinha in the city of Lagoa Santa-MG. After completion of the situational diagnosis, it was found that the unit did not run the program in full Hiperdia, where they were being made only piecemeal actions. From this reality was a plan of intervention conducted by a multidisciplinary team, with support from primary care and coordination of the municipal health department, which conducted a series of health education activities in order to effectuate fully the actions of Hiperdia. Activities occurred from March to December 2013. The results can be seen a satisfactory change in the presence of large number of patients enrolled in the program, including reports of satisfaction by those present, which allowed the reduction of risks related to complications of diabetes and hypertension, coordinating activities promotion and disease prevention, increasing interest and participation of those present, as well as better control of blood pressure and glucose levels, encouraging a change of habits, as in the case of proper nutrition, reducing sedentary lifestyle, tobacco control, the importance of physical activity aimed at a better quality of life. Thus we conclude that the actions of preventive health is multidisciplinary and critical to the effectiveness of the program Hiperdia and should be linked to effective participation of all network professionals to achieve integral action and not fragmented.
