Abstract

Hypertension is currently the main comorbidity afflicting the Brazilian population, being present in one in five Brazilians, according to the World Health Organization (WHO). It has a high prevalence and low rates of control, not only in Brazil but also in other countries. Although there are data showing that there is no age at onset of Hypertension above Systemic, since some factors may favor the incidence of the disease, it is known that the disease is more common in people over the age of 50 years . This study has as its main focus the adhesion of the individual with high blood pressure treatment and follow-up on a health strategy for the Three Points of municipality Family, ESF Dr. Carlos Antônio Fagundes de Sousa. The objective of this study was to prepare a proposal for intervention in order to improve control and adherence of hypertensive patients to medication and non-medication. This is a literature review where scientific production inherent in the theme Hypertension were surveyed. Data System Program of Care Information Basic hypertensive patients registered were raised. Believing in the potential of the Family Health Strategy a proposal for implementation of the intervention project to improve the healthcare for hypertension was presented, in order to improve adherence of hypertensive patients to medication and non-medication by Health teams ESF Family Dr. Carlos Antônio Fagundes de Sousa - Três Pontas.
