| Title: | Efetivação da execução do quarto passo do aleitamento materno pela equipe de enfermagem em uma maternidade de Recife - PE |

| Abstract: | Este estudo de intervenção teve como objetivo efetivar a execução do quarto passo do aleitamento materno pela equipe de enfermagem. Foram utilizados como estratégias para a sua execução seis passos: 1º passo – Convite a Equipe de Enfermagem para a apresentação do projeto; 2º passo – Identificação junto à Equipe de Enfermagem os fatores técnicos, operacionais e estruturais que dificultam a execução do quarto passo; 3º passo - Reunião com a Coordenação de Enfermagem para expor as dificuldades encontradas; 4º passo – Reunião com a Equipe de Enfermagem para a implantação das oficinas; 5º passo – Realização das oficinas de sensibilização e 6º passo – Viabilização de estratégias de Educação Permanente que permitam a execução do quarto passo. Trata-se de um estudo de intervenção realizado através de uma análise qualitativa. Foi desenvolvida no pré-parto, parto e pós-parto da Maternidade Professor Bandeira Filho do município de Recife – PE. Os dados foram coletados através de questionário e observação in loco utilizando um roteiro. Houve a participação de 40 profissionais de enfermagem, sendo, 10 Enfermeiros e 30 Técnicos de Enfermagem, onde se buscou avaliar o nível de conhecimento sobre a temática e as dificuldades técnicas, operacionais e estruturais que dificultam a execução do quarto passo do aleitamento materno. Os resultados demonstraram que nos fatores técnicos apenas 10% dos Enfermeiros não tinham conhecimento e não executavam a prática do quarto passo do aleitamento materno, no entanto, no parto Cesáreo esta prática não é executada. Em relação aos Técnicos de Enfermagem, 50% não tinham conhecimento e não executavam. Em relação aos fatores operacionais foi verificado através de observação in loco que o fluxo de atendimento é adequado para a execução do quarto passo do aleitamento materno, porém, o quantitativo de profissionais é insuficiente. Quanto aos fatores estruturais, foi observado através de um roteiro que revelou que a Maternidade possui estrutura física e ainda mobiliários e equipamentos adequados. Foi verificado ainda que a sala de parto cirúrgico possui baixa temperatura menor que 26°C. A avaliação foi realizada através da observação in loco e da aplicação de um formulário contendo cinco questões referente aos assuntos abordados nas oficinas de sensibilização, ao término a média aritmética de aprendizado dos Enfermeiros foi de 9,6 e os do Técnicos de Enfermagem de 9,5. Conclui-se que a partir da execução deste estudo intervencionista foi possível compreender a importância do contato pele a pele para o binômio mãe e bebê, sensibilizar a Equipe de Enfermagem sobre a temática, contudo, apesar de sua motivação existe um entrave nos partos cirúrgicos onde esta prática não é observada, devido não ser exercida pelos neonatologistas, sendo assim, é primordial a sensibilização também da classe médica. Por fim, avalia-se que a realização de educação permanente para a Equipe de Enfermagem é fundamental para fortalecer a prática mediante a atualização dos seus conhecimentos e sensibilização sobre as ações que favoreçam o quarto passo do aleitamento materno. |
| Abstract: | The purpose of this intervention study was to implement the fourth step of breastfeeding by the nursing team. Six steps were used as strategies for its execution: Step 1 - Invitation to the Nursing Team for the presentation of the project; Step 2 - Identify with the Nursing Team the technical, operational and structural factors that make difficult the execution of the fourth step; Step 3 - Meeting with the Nursing Coordination to expose the difficulties encountered; Step 4 - Meeting with the Nursing Team for the implementation of the workshops; Step 5 - Conducting the awareness workshops and 6th step - Viabilization of Permanent Education strategies that allow the implementation of the fourth step. This is an intervention study conducted through a qualitative analysis. It was developed in the prepartum, delivery and postpartum of Professor Bandeira Filho Maternity in the city of Recife - PE. Data were collected through questionnaire and onsite observation using a road map. A total of 40 nurses participated, including 10 Nurses and 30 Nursing Technicians, in order to assess the level of knowledge on the subject and the technical, operational and structural difficulties that hinder the implementation of the fourth step of breastfeeding. The results showed that in the technical factors only 10% of the nurses were not aware of and did not practice the fourth step of breastfeeding, however, in Cesarean delivery this practice is not performed. Regarding the Nursing Technicians, 50% were not aware and did not perform. Regarding the operational factors, it was verified through on-site observation that the flow of care is adequate for the execution of the fourth step of breastfeeding, however, the number of professionals is insufficient. As for the structural factors, it was observed through a script that revealed that the Maternity has physical structure and also adequate furniture and equipment. It was also verified that the surgical delivery room has a low temperature lower than 26 ° C. The evaluation was carried out through on-site observation and the application of a form containing five questions related to the topics covered in the awareness workshops, at the end the arithmetic average of the Nurses' learning was 9.6 and the Nursing Technicians of 9, 5. It is concluded that from the execution of this interventional study it was possible to understand the importance of skin-to-skin contact for the binomial mother and baby, to sensitize the Nursing Team on the subject, however, despite its motivation, there is an obstacle in surgical deliveries where this practice is not observed, because it is not exercised by neonatologists, and therefore, it is also important to raise awareness among the medical profession. Finally, it is evaluated that the permanent education for the Nursing Team is fundamental to strengthen the practice by updating their knowledge and raising awareness about actions that favor the fourth step of breastfeeding. |
