dc.description.abstract | O objetivo deste artigo é avaliar as evidências disponíveis para cada etapa do tratamento da encefalite autoimune e fornecer opinião de especialistas quando houver falta de evidências. O artigo aborda a encefalite autoimune como uma categoria ampla, em vez de focar em síndromes de anticorpos individuais. Os principais autores da Autoimmune Encephalitis Alliance Clinicians Network revisaram a literatura e desenvolveram o primeiro rascunho. Onde faltavam evidências ou havia controvérsias, uma pesquisa eletrônica foi distribuída a todos os membros para solicitar respostas individuais. Sessenta e oito membros de 17 países responderam à pesquisa. Os corticosteróides sozinhos ou combinados com outros agentes (IG intravenosa ou plasmaférese) foram selecionados como terapia de primeira linha por 84% dos respondedores para pacientes com apresentação geral, 74% para pacientes com convulsões distônicas faciobraquiais, 63% para encefalite NMDAR-IgG e 48,5% para encefalite paraneoplásica clássica. Metade dos respondentes indicou que adicionaria um agente de segunda linha somente se não houvesse resposta a mais de um agente de primeira linha, 32% indicaram adicionar um agente de segunda linha se não houvesse resposta a um agente de primeira linha, enquanto apenas 15% indicaram o uso de um agente de segunda linha em todos os pacientes. Quanto ao agente preferencial de segunda linha, 80% dos respondedores escolheram o rituximabe, enquanto apenas 10% escolheram a ciclofosfamida em um cenário clínico com anticorpos desconhecidos. dc.description.resumo | The objective of this paper is to evaluate available evidence for each step in autoimmune encephalitis management and provide expert opinion when evidence is lacking. The paper approaches autoimmune encephalitis as a broad category rather than focusing on individual antibody syndromes. Core authors from the Autoimmune Encephalitis Alliance Clinicians Network reviewed literature and developed the first draft. Where evidence was lacking or controversial, an electronic survey was distributed to all members to solicit individual responses. Sixty-eight members from 17 countries answered the survey. Corticosteroids alone or combined with other agents (intravenous IG or plasmapheresis) were selected as a first-line therapy by 84% of responders for patients with a general presentation, 74% for patients presenting with faciobrachial dystonic seizures, 63% for NMDAR-IgG encephalitis and 48.5% for classical paraneoplastic encephalitis. Half the responders indicated they would add a second-line agent only if there was no response to more than one first-line agent, 32% indicated adding a second-line agent if there was no response to one first-line agent, while only 15% indicated using a second-line agent in all patients. As for the preferred second-line agent, 80% of responders chose rituximab while only 10% chose cyclophosphamide in a clinical scenario with unknown antibodies.
