dc.description.abstract | INTRODUCTION: Immuno-oncology studies the immune system of cancer patients. In the last decades, immunotherapy has obtained a good response to the treatment of several types of locally advanced and metastatic cancers, by mechanisms of the patient's own immune system to enhance their immune response, acting on the tumor's escape mechanisms. OBJECTIVE: To study the immune mechanisms involved in head and neck squamous cell carcinoma (HNSCC) and their application in immunotherapy through biomarkers. METHOD: Literature review based on references researched at PUBMED, LILACS, MEDLINE, WHOLIS and SCIELO, using the keywords squamous cell carcinoma, head and neck, immuno-oncology, immunotherapy, immunology. RESULTS AND DISCUSSION: The main drugs currently approved for clinical use in patients diagnosed with HNSCC, are Pembrolizumab and Nivolumab, both classified as check point inhibitors (inhibitors of the T-lymphocyte PD-1 receptor), with improved survival and quality of life. Several authors and clinical trials indicate that its indication is conditioned to the dosage of PD-L1 (ligand expressed mainly by tumor cells), which is not a reliable biomarker in the selection of patients. CONCLUSION: Immunotherapy is indicated for patients with locally advanced metastatic HNSCC and has been evolving with PD-1 T-lymphocyte receptor inhibitor drugs, the indication of which is dependent on reliable biomarkers, which must be identified in order to achieve therapeutic success. | pt_BR |
dc.description.resumo | INTRODUÇÃO: Pacientes com carcinoma de células escamosas de cabeça e pescoço (HNSCC) apresentam defeitos na imunidade celular com enfoque nos linfócitos T. Células supressoras são induzidas por HNSCC com consequente comprometimento imunitário. OBJETIVO: Analisar funções celulares imunitárias em pacientes com HNSCC. MÉTODO: Foi realizada imunofenotipagem por citometria de fluxo de células mononucleares sanguíneas de dois grupos de homens adultos (n = 14): grupo 1 - com diagnóstico de HNSCC (boca, laringe e hipofaringe) e grupo 2 - voluntários, saudáveis e sem uso de drogas. Os grupos celulares estudados foram os linfócitos T (CD4, CD8, CD69), linfócitos B (CD19, CD69), neutrófilos (CD11a, CD16, CD66b, HLA-DR) e monócitos (CD14, CD86). RESULTADOS: No grupo 1, houve aumento de linfócitos T CD4+ e de células NK CD56+, bem como de linfócitos B CD19+ expressando o marcador de ativação precoce CD69, além de diminuição de neutrófilos expressando CD11a. Já os números de linfócitos T CD4+ e CD8+ expressando CD69, de monócitos expressando CD14 e CD86 e de neutrófilos expressando CD16, CD66b e HLA-DR não foram diferentes entre os dois grupos. CONCLUSÃO: A presença de carcinoma de células escamosas de cabeça e pescoço desencadeia uma resposta inflamatória sistêmica associada a aumento do número de linfócitos T CD4+, bem como de linfócitos B CD19+, expressando o marcador de ativação precoce CD69, e células natural killer CD56+. Por outro lado, diminui o número total de neutrófilos CD11a. O número de linfócitos T CD8+, de neutrófilos CD16+, CD66b+, HLA-DR e de monócitos CD14+ e CD86+ não foi alterado por essa neoplasia. | pt_BR |
