| Abstract: | Desde o advento dos implantes na Odontologia, sua utilização tem sido aceita por profissionais e pacientes devido ao sucesso cirúrgico e protético. Entretanto, essa modalidade de tratamento não é isenta de complicações. A doença peri-implantar, sobretudo a peri-implantite, é uma das principais causas de falha no tratamento. Apresenta-se geralmente pela perda óssea radiográfica peri-implantar além da fisiológica após carga protética do implante e, clinicamente, com profundidade de sondagem acima de 4 mm, com sangramento quase sempre presente e supuração antes ou após a sondagem. A peri-implantite tem caráter destrutivo, progressivo e irreversível e necessita de intervenção para aumentar a sobrevida dos implantes em função na cavidade oral. Atualmente, não há protocolo padrão para o tratamento da doença. Sabe-se, todavia, que o tratamento não-cirúrgico não é efetivo para descontaminação da superfície do implante e tecidos circunvizinhos. O presente trabalho teve o objetivo de revisar os avanços no tratamento da peri-implantite descritos na literatura dos últimos 5 anos e comparar a eficácia entre eles. Uma pesquisa bibliográfica foi realizada no PubMed e selecionou-se estudos clínicos controlados randomizados, revisões sistemáticas e meta-análises em seres humanos, no idioma inglês, sendo então analisados quanto ao título, resumo e conteúdo, nessa ordem. Houve aproveitamento de artigos referenciados dentre os artigos selecionados, totalizando 41 trabalhos científicos. Diante das informações obtidas, nenhum tratamento foi completamente resolutivo para todos os sinais da doença, entretanto percebeu-se que a utilização de antibióticos e da clorexidina não são efetivas no longo prazo. Ainda não há consenso para um protocolo padrão de tratamento. |
| Abstract: | Since the advent of implants in Dentistry, their use has been accepted by professionals and patients due to their surgical and prosthetic success. However, this treatment modality is not exempt from complications. Peri-implant disease, especially peri-implantitis, is one of the main causes of treatment failure. It usually presents with peri-implant radiographic bone loss, in addition to physiological loss after prosthetic loading of the implant, and clinically with a probing depth above 4 mm, with bleeding almost always present and suppuration before or after probing. Peri-implantitis is destructive, progressive and irreversible and requires intervention to increase the survival of implants functioning in the oral cavity. Currently, there is no standard protocol for the treatment of the disease. It is known, however, that non-surgical treatment is not effective for decontamination of the implant surface and surrounding tissues. This study aimed to review advances in the treatment of peri-implantitis described in the literature over the last 5 years and compare their effectiveness. A literature research was carried out in PubMed and randomized controlled clinical trials, systematic reviews and meta-analyses in humans, in English, were selected, and then analyzed had their title, abstract and content analyzed, in that order. There was use of articles referenced among the selected articles, totaling 41 articles. Based on the information obtained, no treatment has completely resolved all signs of the disease, however it was noticed that the use of antibiotics and chlorhexidine are not effective in the long term. There is still no consensus on a standard treatment protocol. |
