Abstract: | INTRODUÇÃO: O hamartoma mesenquimal hepático é um tumor benigno, mais frequente em crianças. É composto por um tecido fibroso, com quantidade variável de vasos sanguíneos e ductos biliares. Na maioria dos casos, é assintomático, mas pode manifestar com distensão abdominal e dor à palpação. OBJETIVO: Relatar dois casos de tumor hepático benigno na infância. RELATO DOS CASOS: Dois lactentes com grande massa abdominal e exames de imagens mostrando lesão cística hepática com septações em seu interior, sugestivas de hamartoma mesenquimal. Um caso foi submetido a ressecção cirúrgica e o outro, transplante hepático intervivo. Atualmente, ambos se encontram curados e assintomáticos. CONCLUSÃO: O hamartoma mesenquimal deve ser suspeitado em crianças menores de cinco anos, especialmente nos menores de dois anos, com aumento do volume abdominal. Apesar do mesmo diagnóstico, os pacientes foram submetidos a tratamentos distintos para erradicação do tumor. Diante de tumor volumoso, a abordagem cirúrgica deve ser preparada com a possibilidade de inviabilidade da ressecção e necessidade de transplante intervivo durante o procedimento. |
Abstract: | INTRODUCTION: Hepatic mesenchymal hamartoma is a benign tumor more common in children. Comprised of fibrous tissue and with a variable amount of blood vessels and bile ducts, it is asymptomatic in most cases but abdominal distention and pain on palpation can be reported. CASES REPORT: Two infants with large abdominal mass and imaging studies showing cystic hepatic lesions with septations in its interior, suggestive of hepatic mesenchymal hamartoma. One of the patients underwent surgical resection and the other living-donor liver transplantation. Both are deemed healed and currently asymptomatic. CONCLUSIONS: Hepatic mesenchymal hamartoma should be suspected whenever a child younger than 5 years, especially before 2 years of age, presents itself with increased abdominal volume. Even though both patients had the same diagnosis, their treatment strategies were different to eradicate the tumor. When treating a large tumor, the surgical planning must include the possibility of a living-donor transplantation during the procedure. |
