dc.description.abstractIntrodução: Bichectomia promove redução do volume da bochecha pela remoção parcial do bucal almofada gorda. Como qualquer outro procedimento cirúrgico, existem riscos inerentes que podem levar a graves complicações. Para minimizar esses riscos, um novo abordagem usando ultrassonografia em tempo real (EUA) durante o procedimento foi realizado em um caso difícil de bichectomia com variação anatômica na gordura bucal posição. Relato de Caso: Mulher de 25 anos paciente foi encaminhada para bichectomia bochecha esquerda. Foi realizada uma ultrassonografia e uma variação anatômica na posição foi detectou. A camada de gordura bucal esquerda do paciente estava localizado profundamente na área massetérica e próximo relação com ramos da veia facial e a artéria transversa facial. Desde então houve um risco aumentado para realizar a cirurgia, uma bichectomia guiada foi realizada usando ultrassonografia em tempo real para orientar o procedimento inicial incisão, localização e excisão. O procedimento foi realizado sem nenhuma complicação e o sucesso clínico foi avaliado por ultrassonografia no pós-operatório. Conclusão: Os EUA tiveram um desempenho durante o cirurgia possibilitou o acesso seguro e remova a extensão bucal com risco mínimo para o paciente. A ultrassonografia revelou-se útil para demonstrar a relação entre o corpo adiposo bucal e estruturas adjacentes e detectar variação anatômica em sua posição, permitindo um procedimento cirúrgico mais seguro realizado.pt_BR
dc.description.resumoIntroduction: Bichectomy promotes reduction of cheek volume by partial removal of the buccal fat pad. Like any other surgical procedure, there are inherent risks that can lead to severe complications. To minimize those risks, a novel approach using real-time ultrasonography (US) imaging during the procedure was performed on a difficult bichectomy case with anatomical variation in the buccal fat pad position. Case Report: A 25-year-old female patient was referred for bichectomy on her left cheek. An ultrasound scan was performed and an anatomical variation in position was detected. The patient’s left buccal fat pad was located deep in the masseteric area and in close relationship with branches from the facial vein and the transverse facial artery. Since there was an increased risk to perform the surgery, a guided bichectomy was performed using realtime ultrasound imaging to guide the initial incision, location, and excision. The procedure was performed without any complication and the clinical success was assessed by ultrasonography in the postoperative period. Conclusion: The US performed during the surgery made it possible to safely access and remove the buccal extension with minimal risk for the patient. The ultrasound proved to be useful to demonstrate the relationship among the buccal fat pad and adjacent structures and to detect anatomical variation in its position, allowing a safer surgical procedure to be performed.pt_BR
