Abstract: Background: In cases where is not possible to restore and preserve teeths because of an unfavorable prognosis or a complexity of the treatment plan, the extraction of all teeth and the installation of total dentures retained by implants is an option to rehabilitate the patient. Objectives: The aim of this study was to report the clinical outcomes of immediate loaded mandibular overdentures (ILMO) retained by two single implants. Methods: Eighty-one patients were selected for this study. All the maxillary and mandibular teeth were extracted, each patient received maxillary removable complete dentures (ICMD) and 2 implants in the interforaminal region. Overdentures were loaded using ball-attachments when primary stability was achieved (minimum of 60N). Findings: Four patients did not achieve primary stability. Seventy-seven patients received immediate loaded mandibular overdentures. Three of them suffered early loss of one of the implants, and one patient lost both implants within the first month after the surgery. Seventy-three patients completed the three-month osseointegration period without implant loss. They were followed periodically for 36 months, in the older cases and 6 months in the most recent cases. No additional implant loss was observed. Regarding to 77 patients that received immediate loaded overdenture in this study, the implant survival rate was 96,75%. Conclusion: The results showed that immediate loaded overdenture maybe a viable and interesting alternative for the aesthetic and functional rehabilitation of low-income patients with severely damaged dentition.
