Vigilância em saúde da puérpera e do recém nascido do Centro de Saúde Cabana: criação de formulário para auxiliar o profissional de saúde

Abstract

During the first quarter of 2014, there were 17 births enrolled in the team area 02 Center Health Hut. Of these, only three calls from the 5th day were recorded in medical records. This failure results in the loss of important data and content monitoring and surveillance of families. Therefore it was necessary to create a tool for easy and fast filling so that data and evolution during service are not lost. Such instrument, the "Form for Home Visit RN and Postpartum," seeks to guide health professionals in implementing the actions of the 5th on the steps to be followed, in addition to the service record. Therefore, it was necessary to seek theoretical foundation in articles, books and documents of the Ministry of Health and the Department of Health of Minas Gerais and Belo Horizonte. In possession of material, went to the making of the proposed instrument. This instrument is an objective form, complete with space for patient identification. It indicates each step to be done, with space to mark an x when you meet an item, space to write data and assessments, and to attach a document in the medical record, as an evolution of service rendered. With the use of it was possible to record the capture of all infants up to 15 th day of birth born in the catchment area of the team's 02 CS Cabin, early identification of injuries as signs of jaundice in the newborn and maternal emotional instability. Also allowed the verification of the actions of the 5th day and the promotion of exclusive breastfeeding.
