Inserção do grupo de hiperdia na Unidade Básica de Saúde Novo Amazonas

Abstract

Systemic arterial hypertension (SAH) and the Diabetes Mellitus (DM) are part of a class of chronic non communicable diseases (NCDs), being responsible for the highest morbidity and mortality rates in the country, causing significant hospitalization rates and high costs due to complications associated with them. Noteworthy is also that SAH and diabetes are recognized by the Ministry of health as a major public health problem, so much so that this has established guidelines for the monitoring and treatment of individuals affected by them. Therefore, we carried out this study with the goal of developing an action plan for the implementation of HIPERDIA group seeking to improve adherence to self-care of patients with hypertension and diabetes mellitus. A intervention plan followed in the footsteps of situational strategic planning .It Was held literature to theoretically support the work in SciELO, using the keywords: diabetes, hypertension and education. We searched, too, the Ministry of Health documents. As a result of this project, got the creation of the educational group HIPERDIA in Basic Health Unit New Amazonas 10 in 10 days, and along with this, we contribute to develop strategies for prevention, promotion and rehabilitation of the patient with diabetes mellitus and hypertension.
