Abstract

The emergence of the National Program of Family in the 90s aimed at consolidating the Health System and strengthening the primary care through operational rules, decrees and orders. This program shall be considered a strategy focused on the reorientation of care model, with the principles of family and focus on the process of working area bounded scope, identification of clients, scheduled action, community participation. Based on this hypothesis, the study allowed us to know the reality of the problems that are interconnected. Justifying the choice of subject and the action plan to improve the bond between the team and the user through reorganization and planning of actions and services team to ensure better tracking users.Bibliographical research, epidemiological data, log, published articles, electronic, construction of critical nodes by the transaction identification, products, resources, results for the definition and implementation of the action plan investments organs were performed after presentation, release, responsible with predefined period of completion acquired by the organization and community network.Concludes that this project will be a challenge in seeking to establish the reorientation model work, communication and commitment between user and staff, in building a new philosophy geared to the entire population of the area covered by the Basic Health Unit Clarindo Paiva in the city of Corinth / MG.
