A transferência como meio de tratamento do outro e recurso de contenção à passagem ao ato na psicose

Abstract

The aim of this thesis is to investigate, from a clinical case study, in what way the maneuver of the transference in psychosis allows us to bet on its effectiveness as a form of treatment of the Other and resource to contain the passage à l'acte in this structure. Our research proposal came from the treatment of a schizophrenic patient inside of a psychiatric hospital and from the impasses that emerge before the presented intervention. It´s about a serious case, which many different therapeutics actions failed, leaving the professional team, on several occasions, powerless in front of his incessant passage à l'acte. Taking the case as reference, we opted by the construction of the clinical case as a psychoanalysis research method focusing on the monitoring of outputs indicated by the patient himself and on the indications during the conduction of the treatment that allows us confirm the establishment of the transference and its effects on the passage à lacte. Oriented by the reading of Freud and Lacan, we determined the particular characteristic that defines psychosis while clinical structure, noting that in psychosis theres no such thing as the reduction of the enjoyment by the symbolic, which makes it returns over the subject in a massive way as a frightening Other. Consequently, the passage à lacte in this structure configure as an attempt to extract enjoyment from the Other camp as an exit to dealing with the real. In face of that observation, the maneuver of the transference follow as an orientation to the treatment of the Other to block the overflow of the pulsion; in other words, a work modality that sought to regulate the enjoyment of the Other through strategies created by what was possible in this case. The Other's treatment happens when the professional takes a different position in the transference and show the patient an empty Other. This helps the psychotic to tolerate the presence of the Other and it brings to the patient a sort of peaceful feeling in terms of the enjoyment and the modulations of his own acts. In this research, we try to recover theoretical and clinical references that would allow us to establish that: the use of words and the different functions exercised by the psychoanalyst in the transference which allows the patient to give a new name for enjoyment, putting a barrier on the unbridled interpretation of the signs and hindering the development of significance that can precipitate a conclusion in the real by means of the passage à l'acte. Through the construction of this clinical case, we verified that the maneuver of the transference and the use of the psychoanalyst as a multifunctional object presented a fundamental importance for the establishment of a treatment possibility to the passage à lacte in psychosis.
