From this it was possible to trace an intervention project for optimization of care for patients with mental disorders in EFS José Pereira Sobrinho, with the critical nodes of the practice of bad assistance to people with mental disorders, the lack of preparation of health professionals and disorganization the agenda for individual and group care, home visits and family care. This intervention project proposed two subprojects to be developed, namely "Minds preparing minds" and "systematic Service", which until now had the results the practices of continuing and lifelong learning, and the decentralization of monitoring of patients the integration and multidisciplinary approach among health professionals of the Basic Health Unit (UBS), the CAPS, the NASF and psychiatric outpatient clinic. Thus, it was concluded that attention to mental health disorders so prevalent and whose impacts are important, undergoes considerable problems in the SUS, with emphasis on primary care, where we are strategic and technical constraints to determine insufficient assistance and ineffective. There is a need to incorporate systematic actions of mental health care in primary care, and in this work we start the design of the first steps in search of better mental health care in EFS José Pereira Sobrinhopt_BR
