Abstract

CKD is classically defined as an abnormality of structure or function of the kidneys, present for more than three consecutive months and implications for health and quality of one's life, the two main causes of CKD Arterial Hypertension (SAH) and Diabetes Mellitus (DM). (WIKIPEDIA, 2015). Although the criteria for diagnosis of CKD are now much clearer, the proportion of patients with CKD at an advanced stage first seen by a nephrologist is still unacceptable. Early diagnosis and prompt referral to the specialist are essential steps in the management of these patients, as they allow the predialysis education and the implementation of preventive measures that slow or even interrupt the progression to more advanced stages of CKD, as well as decrease morbidity and mortality. CKD patients, the 32 municipalities of the Mucuri and Jequitinhonha Valley region are served in Hiperdia Mining Centre (CHDM) in Teófilo Otoni, since July 2013, with an initiative of the State Secretariat of Health of Minas Gerais (SES / MG ). August / 2013 to June / 2015 program has confirmed 589 patients with CKD followed. The CHDM's mission is to coordinate the structuring of the Network of Health Care for the carrier population Hypertension, Cardiovascular Diseases, Diabetes Mellitus and Chronic Kidney Disease in Minas Gerais, through regionalized and integrated system of health actions. (ALVES, 2013). The aim of this study is to develop an intervention project aimed at identifying the assistance of the profile provided to patients Chronic Renal by Hiperdia Center Gerais - CHDM Polo Teófilo Otoni - MG. I hope the SWOT Matrix management tool to be applied in this interventional project, could lead the multidisciplinary team of CHDM to do a review and careful reflection through the records of CKD patients by identifying what your strengths and weaknesses within the company and its opportunities and threats in the external environment. They may then, after the interventional proposal be accurately identified the challenges to be followed by the team and the progress made to date.
