Busca ativa Hipertensão arterial em uma Unidade de Saúde no Municipio Piranhas-AL: uma proposta de intervenção

Abstract

Hypertension is a chronic disease determined by high levels of blood pressure in the arteries, is one of the main cardiovascular risk factors and the occurrence of stroke and became a serious public health problem in the adult population in Brazil and in the world. Through the use of situational diagnosis was possible to identify the priority problem that accustomed the population registered in the area of 4 health team in the municipality of Piranhas-Alagoas: high blood pressure, and has as objective the active search of the disease, developing an action plan to improve the reception, the mapping and registration of hypertensive patients and those with risk factors triggering and aggravating the disease , increase participation in collective activities, as well as the integration of the team and the population in the preventive actions. Aspects as Situational diagnosis, review of the literature on the theme analyzed, bibliographical references, drawing up the action plan. and important elements offered during the course of specialization were used in this work, the realization of the plan of action, made it possible to realize the importance of using a method of planning as a tool for the Organization of the work process until then made intuitively and automatically, And we hope to get a more effective follow-up of patients with hypertension and improve the quality of life of hypertensive patients
