Plano de ação para controle Hipertensão arterial no Centro de Saúde/CSU em Interlagos no município Divinópolis, Minas Gerais

Abstract

Divinópolis is a city of 230,848 inhabitants, and houses the health center / CSU / Interlagos. This work is due to the significant number of elderly users and hypertensive ascribed to the territory of the team, and these users is looking for staff in emergency situations and alteredpressure levels characterizing the lack of: adequate monitoring, adherence to prescribed drugs and lifestyles incompatible with the disease. It developed this intervention to reduce / improve blood pressure levels among older people in the population ascribed to that territory. Weconducted the situational diagnosis recognizing the main problems faced by the health center staff to plan health actions by the method of Situational Strategic Planning (PES. The information for this intervention were collected by reading the records, some records . team Care Information System basic, medical consultation, discussions with patients during home visits and active observation of the territory the bibliography used were scientific papers available in the database: Virtual Health Library, PUBMED, Virtual Library of the Federal University Minas Gerais, SCIELO, among others. the situation analysis was used the method of the Flash estimate Participative and during the proposed action plan we identified the critical nodes and showed the necessity of realization of this project and the participation of the entire multidisciplinary team, performing changes in the routine work of the team. The coherent and supervised use of antihypertensive medications results in the need to reorganize the team work process expansion of educational activities, active search and home visits. It is also essential to intensify social control and the relationship of health center professionalswith municipal management
