Plano de intervenção diante da elevada prevalência de Hipertensão arterial descompensada na UBS Nossa Senhora de Fátima I Sabará, Minas Gerais

Abstract

This project grew out of needs identified by the team of the Family Health Strategy that works in the community of Our Lady of Fatima i in Sabara, Minas Gerais. The high prevalence of decompensated hypertension led the author of this work, team member, to intervene in the community in a systematic way and together with the team and the community. The intervention aims to reduce this high prevalence. Forthis consisting methodology was adopted two stages: the first in the implementation of strategic planning method for diagnosis of the situation and prioritization of relevant health problems of the population. The second consisted of a literature review to update the scientific knowledge to better intervene in the chosen problem. Next was improved an action plan which aims to ensure the monitoring of users who suffer from high blood pressure, with three strands of main actions: realization of educational lectures for the prevention of Hypertension, training to community workers on the major chronic diseases in the population and finally the active search in each micro area to identify and treat new cases. Relying on the motivation of the players involved in implementing the plan, it is expected that procedurally community adopt a healthier lifestyle and that carriers user Hypertension are encouraged to selfcare
