Intervenção sobre a Hipertensão arterial e risco cardiovascular na Equipe de Saúde da Família Ermida I município Divinópolis - Minas Gerais

Abstract

The Family Health Team Hermitage 1 in Divinópolis / MG provides services in health ascribed SAH to the territory of the team. To reduce the high rate of hypertension, was drawn up a proposal for intervention, encouraging the reduction of modifiable risk factors. This work was proposed after the completion of the situation analysis and the survey of the main health problems existing in the territory. Since then, health actions were planned, using the method of Situational Strategic Planning (PES) and the information collected on staff records, medical consultation and contact with patients during home visits and chips A. The method of the Flash Estimate participative helps to know the living conditions of the territory of the Hermitage 1 staff population, its characteristics, the environment where they live and the way and the way it is distributed in the territory. The nodes identified critical were: The health team work process inadequate to assist hypertensive; Information levels of the hypertensive population about the problem; Customs, habits and lifestyles of the population; of hypertensive exposure to certain modifiable risk factors. The realization of this intervention requires the participation of a multidisciplinary team. For better monitoring through the quantitative and qualitative increase in medical and nursing consultations, pharmaceutical care and nutrition can contribute to the improvement of blood pressure by improving the quality of life of hypertensive patients
