dc.description.abstractCardiovascular diseases (CVD) became a major public health problem, with multifactorial causes which are divided into modifiable and non-modifiable. Acute Coronary Syndrome (ACS) is the most common manifestation corresponding to signs and symptoms that occur with ischemia and outcome of angina or acute myocardial infarction (AMI) among all others CVD. Above all, the evaluation of chest pain remains a major challenge at emergency services. Indeed, analysis of blood markers presents an important tool for the diagnosis of AMI, alternatively other clinical conditions, non-SCA, may lead to changes into electrocardiogram (ECG) ST segment. 1380 medical records of adult patients were evaluated, classified by application of chest pain by using Manchester Protocols. This paper aims to investigate clinical markers, biochimestry and hemostatic qualified to protocol for patients with chest pain at the emergency service, and their association with Acute Coronary Syndrome, by focusing the AMI. Among the patients facing cardiarc arrest there were a significant and more frequent seeking for the emergency services at the first 12 hours after pain chest onset (p=0.011), and 42% of this group seek for help with more than 13hours from the beginning of the chest pain. The markers in serial analyses, myoglobin (p<0.001), CK-MB mass (p<0.001) and cardiac troponin I (p<0.001) and studied population characteristics as average 66.2 ± 13.2 years, male, with high blood pressure (p<0.001), myocardial report on the previous event (p<0.026) and electrocardiogram examination (p<0.001) showed consistent results with previous reports in the literature for infarction patients. By statistical analysis of individual independence a significant relationship between D-dimer markers (p<0.001) and natriuretic peptide type B (p<0.001) with AMI was presented. There was a compatibility between D-dimer and BNP ratio affected to the infarcted ones. High levels of D-dimer were also correlated with the use of anticoagulant and platelet antiaggregant drugs in order to evaluate D-dimer as a promising marker for CVD failure prevention therapy. The decisive statistical model has shown that the following variables were predictors to AMI as the ECG examination on admission at the emergency services, previous AMI historic, myoglobin in the third hour and cardiac troponin I in the sixth hour after admission, with a significant level of 0.05. ECG, Mgb (3H) and TNI (6H) examinations as well as patients cardiac history are variations which allow predicting, in an efficient manner, that AMI in the studied population indicate this work importance to elaborate the risk scores. It is advisable that myoglobin marker be kept on the laboratory analyses by considered that the studied work is related to cardiac disease and may refer to previous condition of recent cardiac arrest, this event on other sorting markers with a higher sensibility to AMI, like troponin, may be still affected. It became evident that a high vulnerability towards the studied population who suffered from cardiac arrest is being the great challenge by the time from the begin of chest pain and emergency services admission, therapeutic failure in patients who take platelet antiaggregant drugs as well as troponin tests that contribute to a faster diagnostic at point-care of patients with this potentially fatal disease. An interpretative analysis of the obtained data do not prioritize the inclusion non-troponin markers on the routine for seeing patients with chest pain once the acquired value to sensibility, specificity and positive predictive value (PPV) that were not acceptable to confirm AMI in addition to add a greater cost. From the begin of the chest pain to emergency services admission there is still a challenge and, to a better understanding of the risk factors associated to the studied population, the clinical patient registrations at medical records need improvement to a greater information safety.pt_BR
dc.description.resumoAs doenças cardiovasculares (DCV) constituem um problema de saúde pública apresentando causas multifatoriais, as quais se dividem em modificáveis e não modificáveis. Entre as DCV, a Síndrome Coronariana Aguda (SCA) é a manifestação mais comum, correspondendo a sinais e sintomas que cursam com isquemia e desfecho de angina ou infarto agudo do miocárdio (IAM). Nesta condição, a avaliação da dor torácica continua sendo um dos maiores desafios nos serviços de urgência. A análise dos marcadores sanguíneos representa uma importante ferramenta para o diagnóstico do IAM, uma vez que outras condições clínicas, não pertencentes à SCA, podem levar às alterações no segmento ST no eletrocardiograma (ECG). Foram avaliados 1380 prontuários de pacientes adultos, classificados mediante aplicação do protocolo de dor torácica utilizando a Triagem de Manchester. O estudo objetiva investigar marcadores clínicos, bioquímicos e hemostáticos elegíveis ao protocolo de dor torácica em pacientes de um serviço de urgência, e sua associação com a síndrome coronariana aguda, com foco no IAM. Entre os pacientes infartados, foram mais frequente a procura pelo serviço de urgência nas primeiras 12 horas do início da dor torácica (p=0,011) e uma parcela de 42% desse grupo procurou pelo serviço com mais de 13 horas do início da dor torácica. Os marcadores em análises seriadas como a mioglobina (p<0,001), CK-MB massa (p<0,001) e troponina cardíaca I (p<0,001) e características da população estudada como idade média 66,2±13,2 anos (p<0,001), sexo masculino (p<0,001), presença de hipertensão arterial (p<0,001), relato de infarto em evento anterior (p<0,026) e leitura do eletrocardiograma (p<0,001) foram estatisticamente significativas na triagem do IAM. Foi apresentada por meio de análise estatística de independência uma relação individual significativa entre os marcadores Dímero-D (p<0,001) e Peptídeo Natriurético tipo B (BNP) (p<0,001) com o IAM. Foi mais frequente a concordância entre Dímero-D e BNP alterados nos casos infartados. Níveis elevados de Dímero-D também foram correlacionados com o uso dos medicamentos antiagregantes plaquetários e anticoagulantes com a finalidade de avaliá-lo como um promissor marcador de falha na terapia de prevenção de DCV. O modelo estatístico final mostrou que algumas variáveis estudadas foram melhores preditoras para o IAM, como a leitura do ECG na admissão no serviço de urgência, o histórico de IAM anterior, a mioglobina na terceira hora e troponina I na sexta hora após a admissão, com nível de significância de 0,05. É possível afirmar que os exames de ECG, Mgb (3H) e TNI (6H), bem como o histórico cardíaco do paciente, são variáveis que podem predizer, de forma eficaz, que o IAM na população estudada indica a importância deste trabalho para a elaboração de scores de risco. Recomenda-se ainda que o marcador mioglobina seja mantido nas análises laboratoriais uma vez que o serviço estudado é de referência cardiológica e pode atender casos de infartos prévios recentes, evento este em que outros marcadores de triagem com maior sensibilidade para o IAM, como a troponina, ainda podem estar alterados. Ficou evidente, ainda uma alta vulnerabilidade para o infarto na população estudada, sendo o grande desafio entre o tempo de início da dor torácica e a admissão na urgência, a falha terapêutica nos casos de pacientes em uso de antiagregantes plaquetários, assim como a contribuição dos testes troponinicos a beira do leito como auxiliares rápidos no diagnóstico desta enfermidade potencialmente fatal. Uma análise interpretativa dos dados obtidos não prioriza a inclusão dos marcadores não troponínicos na rotina de atendimento da dor torácica, uma vez que os valores obtidos para sensibilidade, especificidade e valor preditivo positivo (VPP) não foram satisfatórios para confirmação do IAM, além de agregar maior custo. O tempo de ínicio da dor torácica e a admissão no serviço de urgência ainda é um desafio e, para melhor compreensão dos fatores de risco associados a população estudada, os registro clínico dos pacientes no prontuário médico necessitam ainda de melhorias para uma maior segurança das informações.pt_BR
