Redução de hipertensos idosos descontrolados na população da equipe do município de Barra Longa, Minas Gerais

Abstract

Systemic hypertension (SH) is a multifactorial clinical condition characterized by high and sustained levels of blood pressure (BP). Associate often the functional changes, and structural or of target organs, heart, brain, kidneys and blood vessels and metabolic changes, with consequent increased risk of fatal cardiovascular events and nonfatal (VI HYPERTENSION GUIDELINES, 2010). The high number of patients withchronic diseases using health services repeatedly can reveal, among other factors, inadequate therapeutic follow-up, in addition to increasing demand for health services, favors hospitalization or contributes to extend the duration. This paper aims to draw up a proposal for intervention with a view to reducing elderly hypertensive uncontrolled in Long Bar municipality. A literature review was made in the electronic databases of the Scientific Electronic Library Online (SciELO), Virtual Health Library(VHL), and the Ministry of Health (MOH). The updated studies were selected, with preference for those ten articles published in the last ten years. The action plan was based on the method of Situational Strategic Planning (PES) as Campos, Faria, Santos (2010). It was concluded that the difficulties of changing habits and life are associated with several factors in the case of the elderly population of Long Bar, especially the unfavorable financial condition, cultural and social. Given this reality,the participation of several health professionals, a multidisciplinary approach is of great importance to provide guidance and appropriate assistance regarding the treatment of hypertension, and thus facilitate adherence to antihypertensive treatment and consequently increase control blood pressure.
