Title: Correlação entre comprimento dos telômeros com composição corporal e fatores de risco cardiovascular em idosos com diferentes perfis cognitivos

Abstract: O envelhecimento da população brasileira tem sido significativo ao longo dos anos. Com o aumento da expectativa de vida têm-se observado aumento da incidência de Comprometimento Cognitivo Leve (CCL) e Demência de Alzheimer (DA). Entre as consequências do avanço da idade, as alterações na composição corporal, assim como o aumento dos fatores de risco cardiovascular (RCV) têm sido associados ao comprometimento cognitivo. A presença desses desfechos pode acelerar o encurtamento do telômero que tem sido considerado um marcador do envelhecimento celular. Além disso, a presença de polimorfismos genéticos da Apolipoproteína E (APOE) também tem sido associada a maior conversão de CCL para DA. O objetivo principal do estudo foi elucidar a associação entre comprimento relativo do telômero com fatores relacionados a composição corporal em idosos Controle (GC), com CCL e DA. Como objetivos secundários: caracterizar a relação dos fatores sócio demográficos, genéticos (polimorfismo da APOE) e clínicos (composição corporal, fatores de RCV, etilismo, entre outros) com os diferentes perfis cognitivos e avaliar se o comprimento relativo do telômero se associa aos fatores de RCV. Os idosos foram submetidos a avaliação geriátrica e neuropsicológica para diagnóstico e coleta de dados clínicos. A composição corporal foi feita por método de Absorciometria por dupla emissão de raios-x (DXA). Para análise genética foram coletados 4 ml de sangue periférico em EDTA para extração do DNA por método salino e posterior Reação em Cadeia de Polimerase em tempo real (PCR). Na análise descritiva a amostra tinha prevalência do sexo feminino, com idade mais avançada no grupo DA e menor escolarida no grupo CCL com relação ao GC. Para fatores de RCV o GC teve maior percentual de dislipidemia, embora sem associação com maior RCV. Na parte genética a prevalência de polimorfismo foi observada no grupo DA que também obteve menor comprimento de telômero. Com relação a composição corporal, o Índice de Massa Corporal (IMC) e a circunferência abdominal (CA) foi menor no grupo DA com relação ao GC. O comprimento do telômero se associou apenas a hipertensão (HAS) no CCL e na Regressão Linear com dislipidemia e ao grupo DA. Concluímos que o maior IMC e CA são fatores de proteção no idoso e que a relação inversa do comprimento relativo do telômero com DA é clara. No CCL a variabilidade do tamanho telomérico no grupo pode interferir nos desfechos observados com relação a HAS. Para dislipidemia, a associação observada corrobora com outros estudos. Com relação a composição corporal não houve associação significativa com o comprimento relativo do telômero.
Abstract: The aging of the Brazilian population has been significant over the years. With the increase in life expectancy, there has been an increase in the incidence of mild cognitive impairment (CCL) and Alzheimer's dementia (AD). Among the consequences of advancing age, changes in body composition, as well as increased cardiovascular risk factors (CVR), have been associated with cognitive impairment. The presence of these endpoints may accelerate shortening of telomeres that have been considered a marker of cellular aging. In addition, the presence of apolipoprotein E (APOE) genetic polymorphisms have also been associated with a greater conversion of CCL to DA. The main objective of the study was to elucidate the association between relative telomere length with factors related to body composition in elderly control (CG), with CCL and AD. As secondary objectives: to characterize the relationship between socio-demographic, genetic (APOE polymorphism) and clinical factors (body composition, CVR factors, alcoholism, among others) with the different cognitive profiles and to evaluate if the relative telomere length is associated with the factors of RCV. The elderly were submitted to geriatric and neuropsychological evaluation for the diagnosis and collection of clinical data. The body composition was made by Absorciometry method by dual emission of x-rays (DXA). For the genetic analysis, 4 ml of EDTA peripheral blood were collected for DNA extraction by saline and then realtime Polymerase Chain Reaction (PCR). In the descriptive analysis, the sample had a female prevalence, with a more advanced age in the AD group and a lower level in the CCL group compared to the CG. For RCV factors, GC had a higher percentage of dyslipidemia, although no association with higher RCV. In the genetic part, the prevalence of polymorphism was observed in the AD group, which also obtained a shorter telomere length. Regarding body composition, the Body Mass Index (BMI) and waist circumference (WC) were lower in the AD group than in the CG group. The telomere length was associated only with hypertension (SAH) in CCL and in Linear Regression with dyslipidemia and the AD group. We conclude that the highest BMI and CA are protective factors in the elderly and that the inverse relation of the relative telomere length with the AD is clear. In CCL the variability of telomere size in the group may interfere with the observed outcomes regarding SAH. For dyslipidemia, the observed association corroborates with other studies. Regarding body composition, there was no significant association with the relative telomere length.
