Plano de intervenção para controle do diabetes mellitus 2 na população do bairro Vila Nova em Monte Carmelo, Minas Gerais

Abstract

Monte Carmelo is a Minas Gerais State city located in Alto Paranaíba with 48.096 inhabitants. After the realization of situational diagnosis was chosen the most important problem of research the inadequate medical monitoring of patients with diabetes mellitus type 2. The majority of population dont make adequate follow-up and develop with multiple complications. What is a needed, therefore, establish a model for the adequate medical monitoring that evite inferior quality of life and cronic conditions. The objective this work is elaborate a plan of intervention for glucose control in area of influence of ESF Elias de Morais II, in Monte Carmelo, Minas Gerais. For this work will be used situational strategic planning view to identifying patients with DM2 by means of clinical and laboratorial exams and posterior stratification. The literature review about the theme will be utilize databases of Lilacs andScientific Electronic Library Online. After the implementation of the plan pf action is expected to reduce DM complications and improve regulation of medical consultations according to Primary Attention flow attendance.
