Title: Caracterização do perfil de ativação endotelial e disfunção plaquetária na doença renal crônica em pacientes pediátricos

Abstract: A doença renal crônica (DRC) está associada à elevada morbimortalidade, inclusive por alterações hemostáticas e doenças cardiovasculares, e ao comprometimento na qualidade de vida dos pacientes. Na infância, essas alterações são extremamente relevantes por afetarem não só o curso da doença, como também desencadear comprometimentos na vida adulta. O objetivo do presente estudo foi investigar as funções endotelial e plaquetária em pacientes pediátricos com DRC pré-dialítica e comparar com indivíduos saudáveis, bem como avaliar se os biomarcadores analisados estão relacionados com a doença renal crônica em crianças e adolescentes. O estudo incluiu 95 participantes, sendo 43 pacientes pediátricos com doença renal crônica (DRC) em estágios 1 a 4 em tratamento conservador, e 52 crianças saudáveis (grupo-controle), pareados em idade e sexo. Como marcadores da ativação plaquetária foram determinadas, por citometria de fluxo, a expressão plaquetária de P-selectina (CD62P), GPIIb/IIIa (CD41a), GPIIIa (CD61) e CD40L (CD154), e a percentagem de agregados plaquetas-leucócitos. Os níveis plasmáticos dos biomarcadores de ativação endotelial sE-selectina, fator de crescimento endotelial vascular (VEGF), moléculas de adesão celular vascular (sVCAM-1) e intercelular (sICAM1) foram determinados por ELISA, bem como os biomarcadores receptor de uPA solúvel (suPAR), ADAMTS13 e FVW. As expressões de P-selectina e GPIIIa nos pacientes com DRC foram inferiores ao grupo-controle. A percentagem de agregados plaquetas-monócitos (APM), os níveis plasmáticos de sE-selectina, VEGF, sICAM-1, sVCAM-1, suPAR, FVW e a relação FVW:ADAMTS13 foram mais elevados na doença. Os níveis de FVW e a sE-selectina se apresentaram reduzidos nos pacientes tratados com os antagonistas do Sistema ReninaAngiotensina, refletindo uma proteção endotelial conferida por esses medicamentos. Os resultados obtidos no presente estudo permitem concluir que houve evidências de disfunção plaquetária e ativação endotelial nas crianças e adolescentes com DRC em estágio pré-dialítico; as alterações encontradas estão presentes precocemente na população pediátrica e são promovidas principalmente pela doença renal; as alterações dos marcadores de função plaquetária e endotelial e os níveis de FVW e ADAMTS13 nas crianças e adolescentes com DRC em estágios avançados pré-dialíticos suportam a existência de um estado de hipercoagulabilidade nesses pacientes; a etiologia da DRC (malformações congênitas, glomerulopatias ou outras) não mostrou associação com a alteração dos biomarcadores hemostáticos, de disfunção plaquetária e endotelial; houve correlação entre a expressão dos biomarcadores plaquetários e também entre a produção dos marcadores de ativação endotelial. A redução do RFG foi associada à menor expressão dos receptores plaquetários, à maior formação de APM e aos maiores níveis de marcadores endoteliais. Os biomarcadores de ativação plaquetária e endotelial, quando avaliados em conjunto por meio da regressão logística, mostraram relação com a DRC e com o RFG.
Abstract: Chronic kidney disease (CKD) is associated with high morbidity and mortality, including hemostatic changes and cardiovascular diseases, and compromised patients' quality of life. In childhood, these changes are extremely relevant because they affect not only the course of the disease but also triggering impairments in adult life. The objective of the present study was to investigate the endothelial and platelet functions in pediatric patients with pre-dialytic CKD and compare it with healthy individuals, as well as to evaluate if the biomarkers analyzed were related to chronic kidney disease in children and adolescents. The study included 95 participants, 43 pediatric patients with chronic renal disease (CKD) in stages 1 to 4 on conservative treatment, and 52 healthy children (control group) matched for age and sex. Platelet activation markers expression: Pselectin platelet (CD62P), GPIIb/IIIa (CD41a), GPIIIa (CD61) and CD40L (CD154), and the percentage of platelet-leukocyte aggregates were determined by flow cytometry. Plasma levels of the endothelial activation biomarkers sEselectin, vascular endothelial growth factor (VEGF), vascular cell adhesion molecules (sVCAM-1) and intercellular (sICAM-1), uPA receptor biomarkers soluble (suPAR), ADAMTS13 and FVW were determined by ELISA. Expressions of P-selectin and GPIIIa in patients with CKD were lower than the control group. The percentage of platelet-monocyte aggregates, plasma levels of sE-selectin, VEGF, sICAM-1, sVCAM-1, suPAR, FVW and the FVW: ADAMTS13 ratio were higher in the disease. The levels of VWF and sE-selectin were reduced in patients treated with the Renin-Angiotensin Systems antagonists, reflecting an endothelial protection conferred by these drugs. The results allow to conclude that there was evidence of platelet dysfunction and endothelial activation in pre-dialytic CKD children and adolescents; the alterations are present early in the pediatric population and was promoted by renal disease; the changes in of platelet and endothelial biomarkers function and the levels of VWF and ADAMTS13 in CKD children and adolescents in pre-dialytic stages support the existence of a hypercoagulable state in these patients; the etiology of CKD (congenital malformations, glomerulopathies or others) showed no association with alteration of hemostatic biomarkers, platelet and endothelial dysfunction; there was a correlation between the expression of the platelet biomarkers and also between the production of endothelial activation markers. Reduction of RFG was associated with lower expression of platelet receptors, higher APM formation, and higher levels of endothelial markers. The biomarkers of platelet and endothelial activation, when assessed together by logistic regression, showed a relationship with CKD and with RFG.
