Abstract: | Realizar uma análise detalhada relativa ao papel das trombofiliasgeneticamente determinadas como fatores predisponentes ao aparecimento de paralisia cerebral hemiplégica relacionada a acidente vascular cerebral isquêmico, abordando os principais aspectos conceituais, epidemiológicos e fisiopatológicos. O levantamento de artigos sobre o tema foi pesquisado nas bases de dados Medline, Lilacs, Embase, Cinahl e Cochrane, com palavras-chave acidente vascular cerebral, antitrombina, fator V Leiden, hemiplegia, infarto cerebral, paralisia cerebral, paralisia cerebral hemiplégica, proteína C, proteína S, protrombina, protrombótico, trombofilia, trombose e tromboembolismo. Os limites utilizados foram a faixa etária até 18anos e o período compreendido entre 1998 e 2008. Não houve restrição por idioma de publicação. A paralisia cerebral hemiplégica é uma causa frequente de incapacidade física na criança. Nos últimos anos, o acidente vascular cerebral isquêmico tem emergido entre as principais causas; porém, pouco se c hece sobre os fatores etiológicos envolvidos, suas interações, seus mecanismos fisiopatológicos e sua evolução. Trombofilias geneticamente determinadas são estados de hipercoagulabilidade herdados, que contribuem como possíveis condições predisponentes ao aparecimento desse grave evento comconsequente dano cerebral. O acidente vascular cerebral isquêmico é uma importante causa de paralisia cerebral hemiplégica. Apresenta elevada morbi-mortalidade e forte impacto psicossocial nas crianças afetadas e suas famílias. Sua etiologia é multifatorial, e muitosfatores de risco ainda se encontram indefinidos, com consequente risco de recorrência. Até o presente momento, as pesquisas revelam que as trombofilias geneticamente determinadas têm importância fisiopatológica para o acidente vascular cerebral isquêmico e, consequentemente, a paralisia cerebral hemiplégica. Essa afirmação, porém, não é inequívoca, uma vez que outros trabalhos não conseguiram comprovar os mesmos achados. Resistência à proteína Cativada - fator V Leiden, deficiência de proteína C e mutação no gene do fator II G20210A têm a princípio maior relevância na faixa etária pediátrica, com estudos divergentes na vida adulta. Novas pesquisas nessa área se fazem necessárias objetivando elucidar o real papel dastrombofilias como fatores predisponentes para acidente vascular cerebral isquêmico. A partir disso, poderemos traçar medidas preventivas e terapêuticas, além de adequada orientação eaconselhamento genético. |
Abstract: | The purpose of this study is to perform a detailed analysis about the role of the genetically determined thrombophilias as predetermining factors to the hemiplegic cerebral palsy due to ischemic stroke, approaching the main conceptual, epidemiological andphysiopathological aspects. Articles about such theme have been researched in Medline, Lilacs, Embase, Cinahl, Cochrane databases with the following keywords: ischemic stroke, antithrombin, protein C, protein S, factor V Leiden, prothrombin, hemiplegic cerebral palsy, prothrombotic, thrombophilias, thrombosis, thromboembolism. The limits were subjects up to eighteen years old and the period between 1998 and 2008. No language restriction. Hemiplegic cerebral palsy is a frequent cause for children impairment. Lately, ischemic strokes have emerged amongst the main factors, although little is known about the etiological factors involved, their interactions, physiopathologicalmechanisms and evolution. Genetically determined thrombophilias are hypercoagulability states inherited, which contribute as possible predisponent conditions for the development of such serious event with consequent brain damage. Ischemic stroke is an important cause of hemiplegic cerebral palsy. It shows high morbidity and mortality thus bringing forth strong psychological and social impact on the affected subjects and their families. Its etiology is multifactorial and has severalindefinite risk factors. Currently, the researches demonstrate that thrombophilia have physiopathological importance for the ischemic stroke and hence, hemipleg cerebral palsy. That statement is not absolute, since other studies could not prove the same findings.Resistance to activated protein C - Factor V Leiden, coagulation factor II G20210A mutation and Protein C deficiency show, at first, major relevance in children, but there have been some controversy studies in adults. New researches are necessary to elucidate the actual role ofthrombophilia as predetermining factor of ischemic stroke. Therefore, we should carry preventive and therapeutic actions besides suitable guidance and genetic counseling. |
