Title: Acurácia de enfermeiros na classificação de risco em Unidade de Pronto Socorro de um Hospital Municipal de Belo Horizonte

Abstract: A demanda de atendimento nos serviços de urgência tem aumentadoconsideravelmente nos últimos anos. Como estratégia de organização e humanização da assistência tem-se implantado o Acolhimento com Classificação de risco. Estratégias de acolhimento com protocolos para a classificação de risco têm sido implantadas nos serviços de urgência e no Brasil. Estas têm incluído protocolos para garantir que critérios padronizados de avaliação dos usuários sejam implementados. Este estudo teve como finalidade investigar a acurácia dos enfermeiros na avaliação e classificação de risco estabelecidas no protocoloinstitucional de um hospital municipal de Belo Horizonte, Minas Gerais, pela avaliação dos registros dos Boletins de Entrada (BE); das divergências contidas na avaliação realizada pelos enfermeiros no que se refere à coleta das informações e estabelecimento das prioridades de atendimento; na avaliação da classificação de risco frente à presença, ausência e/ou alteração nos dados vitais e pela determinação da acurácia dos enfermeiros no estabelecimento dos níveis de classificação de risco, pela concordância entre eles e o protocolo institucional. Trata-se de estudo de coorte retrospectivo, com análise de registros de 382 fichas de atendimento dos usuários atendidos no setor de avaliação e classificação de risco, desde a implantação daestratégia de acolhimento em 22 de setembro de 2005 até 22 de setembro de 2007. Realizou-se uma análise descritiva com estabelecimento de freqüência simples e percentual, assimcomo índices de concordância para a determinação da acurácia dos enfermeiros classificadores com relação à classificação atribuída. Para isso, foram calculadas a sensibilidade, a especificidade, o Valor Preditivo Positivo (VPP), o Valor Preditivo Negativo (VPN) e o coeficiente Kappa para cada nível de classificação. Considerando que os registrossão fundamentais para a atribuição do grau de risco ao usuário os resultados apontam uma baixa qualidade dos registros realizados no momento da avaliação do usuário e atribuição do risco. A ausência dos dados registrados pelos enfermeiros, no momento da consulta inicialnão demonstrou influência na classificação de risco realizada mediante o protocolo institucional. Os únicos dados escassos que mostraram uma associação com a discordância na classificação de risco realizada pelos enfermeiros e o protocolo institucional foram os dados vitais. Observou-se uma concordância sofrível a razoável entre os níveis de classificação, representados pelo coeficiente Kappa geral obtido (0,36). Os resultados apontam ainda uma tendência dos enfermeiros em classificar os usuários com graus de risco menos graves do queo estabelecido no protocolo institucional e, em contrapartida, em alguns casos foi observada uma tendência de superestimação do risco, o que pode determinar uma sobrecarga do Pronto Socorro pelo aumento na demanda de atendimento de pacientes que se encontram emsituações clínicas caracterizadas por pouca ou nenhuma urgência. O estudo aponta para a necessidade de treinamento periódico dos profissionais enfermeiros daquele setor no que se refere às avaliações clínicas pautadas no protocolo institucional estabelecido com vistas àmelhoria dos registros e, conseqüentemente da acurácia dos mesmos. Estas estratégias podem representar mais segurança para a equipe multiprofissional e melhora da atenção à saúde dos usuários no que concerne o atendimento de urgência.
Abstract: The demand for care in the emergency services has increased considerably in recent years. As a strategy for organizing and humanization of the assistance has been implanted with the home-risk rating. Strategies with the host protocols for the classification of risk havebeen deployed in the emergency services and Brazil. These have included protocols to ensure that standardized criteria for evaluation of the users are implemented. This study aimed at investigating the accuracy of the nurses in the assessment and classification of risk inestablished institutional protocol of a municipal hospital in Belo Horizonte, Minas Gerais, for the evaluation of the records of Bulletins of Entry (BE), the differences in the evaluation made by nurses regarding the collection of information and establishment of priorities for care; in assessing the vital data and by determining the accuracy of the nurses in the establishment of classification levels of risk, by agreement between them and the institutional protocol. It is retrospective cohort study, with analysis of records of 382 sheets of service users in the sector attended the evaluation and classification of risk, since the implementation of the strategy to host on 22 September 2005 until September 22, 2007. There was a descriptive analysis with the establishment of simple frequency and percentage, as well as indexes of agreement to determine the accuracy of nurses binders with respect to the classification assigned. For this, we calculated the sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and Kappa coefficient for each level of classification. Considering that the records are central to the award of the degree of risk to the user the results indicate a low quality of the records made at the user's assessment and allocation of risk. The lack of data recorded by nurses at the time of the initial consultation showed no influence on the classification of risk held by institutional protocol. The only data that showed a limited association with the discrepancy in the classification of risk undertaken by nurses and institutional protocol were the vital data. There was a correlation between poor to reasonablelevels of classification, represented by the general Kappa coefficient obtained (0.36). The results also indicate a trend of nurses in classifying the degrees of risk users with less serious than that provided in institutional protocol and, in return, in some cases there was a tendencyto overestimate the risk, which can determine an overload of the Emergency Department by the increase in demand for care of patients who are in clinical situations characterized by little or no urgency. The study points to the need for periodic training of professional nurses thatsector with regard to clinical evaluations based on institutional protocol established to improve the records and consequently the accuracy of them. These strategies may represent more security for the team and improvement of health care users with regard to the care ofemergency.
