Title: Sarcopenia, mobilidade funcional e nível de atividade física em idosos ativos da comunidade.

Abstract: a sarcopenia é uma das mais significantes mudanças fisiológicas associadas ao envelhecimento e parece ser a principal responsável pela perda de mobilidade funcional em idosos. Os objetivos deste estudo foram explorar força e potência muscular de membros inferiores e as medidas de circunferência de panturrilha (CP), força de preensão palmar (FPP), mobilidade funcional e nível de atividade física em idosos ativos e comunitários, de ambos os gêneros, com idades entre 65-69, 70-79 e 80 e mais anos e identificar o melhor parâmetro clínico para rastreamento de redução de função muscular de membros inferiores em idosos. Materiais e Métodos: 81 idosos comunitários (42 mulheres e 39 homens) participaram do estudo. Foram avaliadas a velocidade de marcha (Kit Multisprint), FPP (dinamômetro Jamar), função muscular de quadril, joelho e tornozelo (dinamômetro isocinético Biodex System 3 Pro), nível de atividade física (Perfil de Atividade Humana) e CP (fita métrica). Procedeu-se a análise estatística com os testes de Análise de Variância, correlação de Pearson e curva ROC. O nível de significância de 0,05 foi considerado. Resultados: os idosos de 80 e mais anos apresentaram valores significativamente menores que os de 65-69 para CPD (34,93 vs 37,73,6), velocidade de marcha habitual (VMH) (1,10,2 vs 1,20,2) e máxima (VMM) (1,40,3 vs 1,70,3), FPP (23,87,5 vs 31,810,3), média de pico de torque e potência média de quadril, joelho e tornozelo (p<0,05). O nível de atividade física não apresentou diferença significativa entre os grupos etários. A maioria das diferenças de médias do grupo de 70-79 anos não foi significativa. Os parâmetros de função muscular apresentaram correlações moderadas com a VMH, VMM e FPP (p<0,05) e correlações baixas com a CP e nível de atividade física (p<0,05). A análise da curva ROC sugeriu o ponto de corte de FPP de 14,51 Kgf para rastreamento de redução de função muscular nas mulheres idosas. Conclusão: este estudo encontrou associação entre função muscular de membros inferiores, FPP e VMM; demonstrou haver diminuição desses parâmetros com o avançar da idade e sugeriu a possibilidade de rastreamento da função muscular de membros inferiores por meio da FPP. Isto indica a importância de otimização da força e potência muscular nos programas de prevenção e reabilitação para manter a mobilidade funcional de idosos comunitários e a necessidade de mais investigações para introduzir a ferramenta de FPP nas avaliações de rotina.
Abstract: sarcopenia is one of the most significant physiological changes related to aging and seems to be the main cause of functional mobility loss in the elderly. The objectives of this research were to explore lower extremity muscle strength and power, calf circumference (CC), handgrip strength (HG), functional mobility and level of physical activity among age groups of active older people (men and women) and to identify the best parameter for screening the muscle function loss in elderly. Materials and Methods: 81 community-dwelling elderly (42 women and 39 men) participated. Walking speed (Multisprint Kit), HG (dynamometer Jamar), muscle function of hip, knee and ankle (isokinetic dynamometer Biodex System 3 Pro), level of physical activity (Human Activity Profile) and CC (tape measure) were evaluated. Variance analysis, Pearsonss correlation and ROC curves were used for statistical analysis. Statistical significance was accepted if the two-tailed P value was < 0.05. Results: elderly over the age of 80 showed lower values than the 65-69 age group for right CC (34.93 vs 37.73.6), habitual (1.10.2 vs 1.20.2) and fast (1.40.3 vs 1.70.3) walking speed, HG (23.87.5 vs 31.810.3), average peak torque and average power of hip, knee and ankle (p<0.05). The level of physical activity did not show differences among age groups. The majority of the differences of averages of the 70-79 age group was not significant. In analysis of correlations, muscle function parameters showed moderate significant correlations with walking speed and HG, and fair degree of relationship with CC and level of physical activity (p<0.05). The ROC curve analysis suggested a cutpoint of 14.51 Kgf for screening muscle function loss in elderly women. Conclusion: This study showed an association between muscle function, HG and fast walking speed as well as the decreasing of these parameters with aging and suggested a possible screening of reduction of muscle function of lower extremities through HG. These results indicated that muscle strength and power optimization are needed on prevention and rehabilitation programs to maintain functional mobility in community-dwelling elderly. In addition, more research is necessary to introduce hand grip performance on routine assessment.
