Paracoccidioidomycosis (PCM) is caused by thermodimorphic fungi, which currently comprise two species endemic in Latin American countries: Paracoccidioides brasiliensis and Paracoccidioides lutzii. The latter occurs in the central and northern regions of Brazil. The differentiation of species is only possible with molecular methods.Oral manifestations of PCM are strongly related to gingival involvement. treatment with itraconazole 200 mg/day, the patient presented significant clinical improvement and remained in outpatient follow-up. We observed that identification of oral lesions and analysis of patient’s clinical and personal information were fundamental for early diagnosis of PCM. The gold standard for the diagnosis of PCM is the histological analysis from biopsy that shows the fungal elements suggestive of Paracoccidioides sp., but identification of the fungus can also be performed by the Periodic Acid Schiff method or impregnation with silver in the exfoliative cytology from the area affected by the lesions or from sputum; serological tests with specific.
