Abstract: | A área de abrangência do Programa de Saúde da Família Alto São Jacinto é composta por 855 famílias, totalizando 2455 pessoas atualmente, e é dividida em 10 micro-áreas. Um dos problemas de saúde mais comuns atendidos é a Hipertensão Arterial Sistêmica (HAS) pelo que a atenção primária à saúde tem um papel fundamental na promoção, prevenção, diagnóstico e tratamento. Na atenção primária à saúde é muito importante o controle da hipertensão arterial sistêmica ocasionando mudanças no estilo de vida e favorecendo a adesão ao tratamento e o auto cuidado. Este trabalho tem como objetivo elaborar um plano de ação para aumentar a adesão ao tratamento e melhorar o acompanhamento por parte da equipe de saúde da família buscando prevenir as complicações da Hipertensão Arterial Sistêmica na Estratégia de Saúde da Família no Alto São Jacinto de Teófilo Otoni, Minas Gerais. Inicialmente foi realizado para identificar o diagnóstico situacional para identificar os problemas relativos à comunidade por meio do método de estimativa rápida. Em seguida, foi feita uma revisão de literatura sobre o tema e elaborado o projeto de intervenção. As ações do projeto incluem capacitação para a equipe de saúde e ações educativas com os pacientes e pessoas da comunidade, abordando a problemática de hipertensão arterial. Espera-se que ocorra mudança de condutas em relação ao conhecimento de hipertensão arterial, seu risco e adesão ao tratamento, melhorando o estado de saúde dos pacientes que participarão do projeto. |
Abstract: | The scope of the Alto São Jacinto consists of 855 families, totaling 2455 people today, and is divided into 10 micro-areas. One of the most common health problems addressed is Systemic Arterial Hypertension. Primary health care plays a key role in promotion, prevention, diagnosis and treatment. In APS, it is very important to control hypertension, causing changes in lifestyle and favoring adherence to treatment and self-care. This study aims to elaborate a plan of action to increase adherence to treatment and to improve the follow-up by the family health team in order to prevent the complications of Systemic Arterial Hypertension in the Alto São Jacinto de Teófilo Otoni / Minas Gerais. Initially, it was carried out to identify the situational diagnosis to identify the problems related to the community through the method of rapid estimation. Afterwards, a review of the literature on the subject was made and the intervention project was elaborated. The actions of the project include training for the health team and educational actions with the patients and people of the community, addressing the problem of hypertension. It is expected that behavior change will occur in relation to the knowledge of arterial hypertension, its risk and adherence to the treatment, improving the health status of patients who will participate in the project. |
