Abstract: | O carcinoma hepatocelular (CHC) é responsável por mais de 90% das neoplasias malignas primárias do fígado. Doenças hepáticas crônicas predispõem ao CHC, promovendo o desenvolvimento de fibrose, cirrose e alterações na transdução de sinal intracelular. Várias funções hepáticas são reguladas pelo cálcio intracelular (Ca2+), incluindo proliferação e morte celular. O receptor de 1,4,5-trifosfato de inositol (ITPR) é o canal exclusivo de liberação intracelular de Ca2+ no fígado. À luz do fato de que a expressão de ITPR3, uma isoforma ausente em hepatócitos normais, é um evento comum na patogênese e desenvolvimento do CHC, nossa hipótese é que os tumores estabelecidos mantêm sua expressão, contribuindo para manutenção. Aqui, foram avaliadas 53 amostras de fígado de CHC e dados clínicos de pacientes afetados por três diferentes etiologias: vírus da hepatite C (HCV), doença hepática alcoólica (DHA) e cirrose criptogênica (CC). Hematoxilina e Eosina (H & E), para contagem de figuras mitóticas, e imuno-histoquímica para coloração de ITPR3 e ssDNA, foram utilizados para avaliar a intensidade da expressão deste canal de cálcio e perfil apoptótico, respectivamente. A correlação entre ITPR3 e parâmetros clínicos foi avaliada comparando as médias da intensidade da coloração ITPR3 no tumor e no parênquima cirrótico com os parâmetros clínicos e percentagem de apoptose ou mitose. Os resultados mostraram que a intensidade da coloração de ITPR3 se correlaciona inversamente com os índices mitóticos e com a porcentagem de apoptose em todas as etiologias de CHC analisadas. Além disso, o índice mitótico correlacionou-se significativamente com a apoptose. Juntos, esses dados mostram um evento intracelular compensatório subjacente à hepatocarcinogênese, que envolve ITPR3, onde mais mortes devido à apoptose levam à proliferação e manutenção do tumor. Portanto, concluímos que a expressão de ITPR3 é um evento comum em todo o processo de hepatocarcinogênese ao estabelecimento de tumores, influenciando a proliferação e a morte celular por apoptose. |
Abstract: | Hepatocellular carcinoma (HCC) accounts more than 90% of primary malignant neoplasms of the liver. Chronic liver diseases predispose to HCC, promoting the development of fibrosis, cirrhosis and changes in intracellular signal transduction. Several liver functions are regulated by the intracellular calcium (Ca2+), including proliferation and cell death. Inositol 1,4,5triphosphate receptor (ITPR) is the exclusive channel of intracellular Ca2+ release in the liver. In light of the fact that ITPR3 expression, an isoform absent in normal hepatocytes, is a common event in pathogenesis and development of HCC, our hypothesis is that established tumors maintain its expression, functioning in its progression. Here, 53 HCC liver samples and clinical data from patients affected by three different etiologies were used: hepatitis C virus (HCV), alcoholic liver disease (ALD) and cryptogenic cirrhosis (CC). Hematoxylin and Eosin (H&E), for counting mitotic figures, and immunohistochemistry for staining ITPR3 and ssDNA, were used to evaluate the intensity of this calcium channel expression and apoptotic profile, respectively. The correlation between ITPR3 and clinical parameters was evaluated by comparing the means of the ITPR3-staining intensity in the tumor and in the cirrhotic parenchyma with the clinical parameters, and percentage of apoptosis or mitosis. The results showed that the intensity of ITPR3 staining inversely correlates with the mitotic indexes, and with percentage of apoptosis in all HCC etiologies analyzed. In addition, the mitotic index significantly correlated with apoptosis. Together, these data show a compensatory intracellular event underlying hepatocarcinogenesis which involves ITPR3, where more deaths due to apoptosis lead to proliferation and tumor maintenance. Therefore, we conclude that ITPR3 expression is a common event throughout the process of hepatocarcinogenesis to tumor establishment, influencing proliferation and cell death by apoptosis. |
