Abstract: | A via aérea artificial é um tubo inserido na via aérea por via nasal, oral ou transtraqueal que permite a passagem dos gases respiratórios com o objetivo de manter a ventilação pulmonar adequada. As vias aéreas artificiais mais utilizadas são os tubos orotraqueais (TOT) e as cânulas de traqueostomia (CT). Estes geralmente possuem um balonete em sua extremidade inferior para evitar movimentação do tubo e refluxo do conteúdo gástrico para o trato respiratório inferior. A lesão da mucosa traqueal esta diretamente relacionada à presença do balonete, e é influenciada pela duração da intubação e pela pressão na parede lateral da traquéia. A mensuração dessa pressão é realizada diariamente e mantida entre 20 e 34 cmH2O ou 15 e 25 mmHg. O objetivo do presente estudo foi realizar uma revisão da literatura e avaliar a pressão do balonete. Foram selecionados 3 estudos experimentais e 1 estudo quaseexperimental, 1 tese de doutorado e 1 estudo retrospectivo. Os estudos mostraram que é necessário estabelecer uma rotina de mensurações da pressão intrabalonete bem como manter a pressão dentro dos valores normais. |
Abstract: | The artificial airway is a tube inserted into the airway through the nose, mouth or transtracheal that allows the passage of respiratory gases in order to maintain adequate ventilation. The orotracheal tubes (TOT) and the tracheostomy tube (CT) are the artificial airways most widely used. These usually have a cuff at its lower end to prevent movement of the tube and reflux of gastric contents into the lower respiratory tract. The tracheal mucosa injury is directly related to the presence of tubes with cuff, and the injury is influenced by the duration of intubation and by the pressure on the lateral wall of the trachea. The measurement of this pressure is performed daily and maintained between 20 and 34 cmH2O or 15 and 25 mmHg. The objective of this study was to do a literature review about the cuff pressure. We selected four articles (3 experimental studies and a quasi-experimental study) and a doctoral thesis (retrospective study). The studies showed that it is necessary to establish a routine pressure measurements intracuff well as maintaining the pressure within normal values. |
