Abstract: | Pacientes com DM1 podem apresentar condições clínicas desencadeadoras de alterações em seu perfil lipídico, e considerando a importância do controle dos fatores de risco cardiovasculares nestes pacientes, objetivou-se avaliar fatores de risco para dislipidemia, bem como sua associação com o perfil lipídico em pacientes com DM1. Os 125 pacientes incluídos neste estudo foram classificados de acordo com presença ou ausência de diferentes fatores de risco para dislipidemia. O colesterol total e os triglicérides foram determinados pelo método enzimático colorimétrico, o colesterol HDL pelo método de precipitação; a HbA1c por imunoturbidimetria, assim como a albumina urinária, devidamente corrigida pela creatinina urinária, dosada por método enzimático. Variáveis de distribuição normal foram comparadas pelo ANOVA e Teste t de Student e as que não apresentaram distribuição normal foram comparadas pelo Teste Kruskall Wallis e pelo Teste Mann-Whitney U. Pela análise dos fatores de risco investigados, verificou-se que a hipertensão, albuminúria, o sedentarismo, o tabagismo, o uso de estatinas, a idade e a duração do diabetes estão associados com aumento apenas dos níveis de triglicérides. A hiperglicemia está associada com o aumento do colesterol LDL e do colesterol total; já o hipotireoidismo não está associado com alterações significativas no perfil lipídico, enquanto que o IMC está associado com aumento do colesterol total, colesterol LDL e triglicérides, além da redução do colesterol HDL. Em função do maior risco cardiovascular na população em estudo, o controle destes fatores de risco assume significativa importância na prevenção do desenvolvimento de perfis mais aterogênicos, bem como das complicações associadas. |
Abstract: | Patients with DM1 may present clinical conditions triggeringalterations in their lipid profile, and considering the importance of controlling cardiovascular risk factors in these patients, the objective was to evaluate the presence of risk factors for dyslipidemia in type 1 diabetic patients, as well as their association with their lipid profile. The 125 patients included in this study were classified according to the presence or absence of various risk factors for dyslipidemia. Total cholesterol and triglycerides were determined by enzymatic colorimetric method, HDL cholesterol by precipitation method; HbA1c by immunoturbidimetry, as well as urinary albumin, which was then corrected by urinary creatinine, measured enzymatically. Normal distribution variables were compared by ANOVA and Student t-test and those without normal distribution were compared by the Kruskall Wallis test and the Mann-Whitney U test. Analyzing the investigated risk factors, it was found that hypertension, albuminuria, sedentary lifestyle, smoking, the use of statins, age and duration of diabetes are associated with increased triglyceride levels. Inadequate glucose control is associated with increased LDL cholesterol and total cholesterol; and hypothyroidism is not associated with significant changes in lipid profile, while BMI is associated with increased total cholesterol, LDL cholesterol and triglycerides, and reduced HDL cholesterol. Due to the increased cardiovascular risk in this population, the control of these risk factors assumes significant importance in preventing the development of more atherogenic profiles, as well the associated complications. |
