Abstract: Ossifying fibroma (OF) is included in the group of fibro osseous lesions, due to the bone substitution by fibrous tissue, with fibroblasts and islets of immature bone; in fact, it represents a true neoplasm with significant increasing potential. Clinical and imaging aspects can contribute to the distinction between OF and fibrous dysplasia. A 60-year-old black woman presented with com plaints of swelling and pain in the posterior region of the left maxilla with 3 years of evolution. Clinically, osseous exposure was ob served in the related region. Computerized tomography analysis revealed a well-defined hyperdense lesion circumscribed by a hypodense halo, with extension from the first to third molar. A pre sumptive diagnosis of OF was established. Surgical removal under local anesthesia was performed easily because the specimen ap peared encapsulated. Histopathology analysis confirmed a fibro osseous lesion. Clinical and histopathological aspects rendered the diagnosis of ossifying fibroma. Healing was uneventful with no signs of recurrence.
