Abstract: | As lesões esportivas são multifatoriais e resultam da interação complexa entre os fatores intrínsecos e extrínsecos. Os estudos que avaliam os fatores de risco para as lesões esportivas assumem a existência de uma relação direta entre exposição e desfecho e a interação entre os múltiplos preditores é avaliada pela soma dos fatores de risco. Esses estudos, baseados neste modelo tradicional, apresentam resultados inconsistentes, pois não consideram as interações complexas da lesão. Essa complexidade da lesão esportiva não reside na soma dos fatores de risco, mas sim nas interações entre os múltiplos fatores relacionados com a demanda imposta sobre o atleta e sua capacidade de lidar com a energia mecânica durante a prática esportiva. Além disso, o aumento da demanda em determinado tecido pode ter origem em estruturas anatomicamente distantes ao tecido em risco de lesão, devido a interdependência entre os segmentos corporais, que é inerente ao sistema musculoesquelético. Fatores relacionados a pelve, quadril e pé (não locais) podem contribuir para o aumento da demanda de energia na região da coxa e assim, podem influenciar a ocorrência de lesão muscular nessa região. Esses fatores nãolocais são pouco estudados acerca da etiologia da lesão muscular em atletas jovens de futebol. Dessa forma, foram desenvolvidos dois estudos na presente tese. O primeiro estudo foi um ensaio teórico com o objetivo descrever o modelo relacional entre capacidade e demanda para o entendimento das lesões esportivas. O segundo estudo foi desenvolvido com o objetivo de investigar as contribuições de fatores não locais relacionados ao aumento da demanda na região da coxa que podem influenciar a ocorrência de lesões musculares em atletas jovens de futebol. Para o segundo estudo foram avaliados 102 atletas de futebol e as lesões foram registradas ao longo de uma temporada. A árvore de classificação e regressão utilizada revelou que fatores relacionados a pelve, quadril e pé podem contribuir para o aumento de demanda na região da coxa. Interações do torque baixo dos rotadores lateral do quadril, assimetria de estabilidade pélvica e assimetria do mau alinhamento do antepé foram identificadas na ocorrência de lesão muscular na coxa. Adicionalmente, torque alto dos rotadores laterais do quadril parece ter efeito protetor para essa lesão. Os resultados da presente tese demonstraram a influencia de fatores não locais na ocorrência de lesão muscular na coxa em atletas jovens de futebol. |
Abstract: | Sports injuries are multifactorial and result from complex interactions between intrinsic and extrinsic risk factors. Studies assessing risk factors for sports injuries assume a direct relationship between exposure and outcome and the interactions between multiple predictors are evaluated by the sum of risk factors. Studies based on the traditional model described above have found inconsistent results because the model does not consider the complex nature of sports injuries. Specifically, it is not as simple as the sum of risk factors. Complex interactions are the influence (interdependence) between multiple factors related to the athletes capability of dealing with the mechanical energy (demands) originated by the sport. Additionally, the increased demand in a tissue could originate from anatomical structures distant from the tissue at risk. This fact could be explained due to interdependence between the body segments, which is inherent in the musculoskeletal system. Factors related to the pelvis, hip and foot (non-local) may contribute to the increase of energy demand in the thigh area and thus, can influence the occurrence of thigh muscle strain. These non-local factors are poorly investigated in studies about muscle injury aetiology in youth soccer players. Accordingly, this thesis outlines 2 studies. The first study was a theoretical paper and the objective was to describe the capacity and demand model for the understanding of sports injuries. The second study was developed in order to identify the contribution of non-local factors related to increased demand in the thigh area that may influence the occurrence of muscle strain in youth soccer players. For the second study, 102 soccer players were assessed and injuries were recorded over 10 months. Classification and regression tree analysis revealed that factors related to the pelvis, hip and foot contribute to the increased demand in the thigh. Interactions among hip lateral rotators isometric torque, asymmetry of pelvic stability and asymmetry of forefoot alignment were found to be related to the occurrence of thigh muscle strain. In addition, high values of hip lateral rotators torque appear to have a protective effect in this injury. The results of this thesis demonstrate the contribution of non-local factors to the occurrence of thigh muscle strain in youth soccer players. |
