Title: Reabsorções radiculares externas após o reimplante de dentes permanentes avulsionados: ocorrência e associação com fatores demográficos, clínicos e genéticos

Abstract: As Reabsorções Radiculares Externas (RRE) representam a complicação mais frequente da cicatrização periodontal de dentes reimplantados, podendo levar à perda do elemento dental. São classificadas em inflamatórias e por substituição e, embora apresentem quadros histopatológicos e manifestações clínicas distintas, poucos estudos investigaram seus determinantes clínicos e demográficos específicos. O presente estudo teve como objetivo avaliar o papel dos fatores demográficos, clínicos e genéticos na prevalência e extensão das Reabsorções Radiculares Externas (RRE) após reimplante de dentes permanentes avulsionados. Dados clínicos e radiográficos de 208 pacientes atendidos na Clínica de Traumatismos Dentários da FO-UFMG (CTD-FO-UFMG) portadores de 269 dentes permanentes reimplantados foram coletados de seus prontuários e avaliados quanto à associação da atividade de RRE na consulta de início do tratamento endodôntico radical (TER). Para estudo dos efeitos dos fatores genéticos na atividade de reabsorção inicial, os pacientes foram genotipados para polimorfismos nos genes que codificam para as citocinas IL-10 (rs 1800896), RANK (rs 8086340), RANKL (rs 2277438) e OPG (rs 2073618) pelo método de RT-PCR. Para verificar quais variáveis exerciam influência no Índice de Reabsorção, foi ajustada uma Regressão Marginal Log-Linear (Poisson). Os resultados obtidos demonstraram que o índice de RREI em pacientes com menos de 11 anos foi 50% maior quando comparada com aquela dos pacientes maiores de 11 anos no momento do trauma (p<0,001). Quando considerado o ponto de corte de 16 anos, o efeito da idade foi mais pronunciado demonstrando maiores índices de reabsorção tanto de RREI quanto de RRES nos pacientes menores de 16 anos que nos pacientes maiores de 16 anos no momento do trauma. Os resultados demonstraram que a demora para o início do tratamento endodôntico após o reimplante aumentou o risco de ocorrência de REE e foi diretamente proporcional ao aumento nos índices de reabsorção. Para estudar o efeito da idade e das variáveis clínicas e genéticas no tipo de RRE observada no início da terapia endodôntica, realizou-se regressão logística multivariada no modelo de GEE. A análise dos genótipos mostrou que a presença da variante alélica C+ (CC+CG) no SNP rs 2073618 do gene que codifica para OPG apresentou associação significativa com o tipo de RRE inicial (p=0,022), mas não influenciou a extensão da reabsorção. Concluiu-se que a ocorrência e os índices de reabsorção inicial na presente amostra estiveram fortemente influenciados pelo tempo decorrido entre o reimplante e o inicio do TER e pela idade do paciente no momento do trauma. Em contrapartida, fatores genéticos apresentaram associação significativa com o tipo de reabsorção observada, entretanto estes não influenciaram na extensão da mesma.
Abstract: External root resorption (ERR) is a serious complication following replantation and its progressive inflammatory and replacement forms are significant causes of tooth loss. This retrospective study aimed to evaluate the factors related to the occurrence of Inflammatory External Root Resorption (IERR) and Replacement External Root Resorption (RERR) shortly after permanent tooth replantation in patients treated at the Dental Trauma Clinic at the School of Dentistry of Federal University of Minas Gerais (DTC SD FUMG) in Belo Horizonte, Brazil. Case records and radiographs of 269 teeth from 208 patients were evaluated for the presence, type, and extension of ERR and tested for their association with age, clinical factors related to the management and acute treatment of the avulsed tooth and the presence of SNPs rs1800896 in the IL-10 (rs 1800896), RANK (rs 8086340), RANKL (rs 2277438) e OPG (rs 2073618). A multivariate Poisson Regression in the GEE model was performed to teste the effect of age, clinical and genetic factors in the occurrence and extention of ERR. The patients age at the moment of trauma and time elapsed between replantation and the onset of endodontic therapy had a marked effect on the ERR prevalence and extension. The progression rate of IERR was 50% higher in patients younger than 11 years when compared to that of patients older than 11 years at the time of trauma (p<0.001). When considered the cutoff point of 16 years, the age effect was more pronounced showing highest rates of reabsorption of both RREI as RRES in patients under 16 years than in patients over 16 years at the time of trauma. The results showed that the time elapsed between replantation and the onset of endodontic therapy progressively affected the prevalence and severity of ERR and was directly proportional to the increase in rates of reabsorption. To study the effect of age, clinical and genetic variables in the type of ERR observed at the onset of endodontic therapy a multivariate logistic regression model in the GEE model was performed. The presence of the allelic variant C + (CC + CG) in SNP rs 2073618 of the gene encoding OPG was significantly associated with the occurrence of IERR (p = 0.022). It was concluded that while demographic and clinical factors did not influence the type of RRE after replantation of permanent teeth, the occurrence and the indexes were strongly influenced by the time elapsed between the replantation and the beginning of the endodontic treatment and the age of the patient time of injury. On the other hand, genetic factors were significantly associated with the type of resorption observed but did not affect.
