razil is entering its fifth decade living with HIV/AIDS and enjoys international prestige for its pioneering responses to the epidemic. The unprecedented mutual approach between the State and civil society established the pillars for the construction and implementation of preventive and assistance policies based on inclusive principles and the recognition of diversity, secularism of the State and strengthening of democracy. Despite the progress achieved during the period, the issue of adherence to antiretroviral therapies by people living with HIV remains one of the main epidemiological challenges faced by researchers studying the topic. It is a complex and multi-determined phenomenon due to its social, political and subjective particularities. The latter reflect an atmosphere of strong emotional intensity, which simultaneously encompasses sensitive themes such as sexuality, stigma and death. The urgency of psychological suffering in diagnostic circumstances contextualizes the carrying out of this research, which seeks to investigate, through the psychoanalytic theoretical framework in dialogue with feminist and queer theories, the traumatic repercussions of receiving a primary diagnosis for HIV on the psyche of male patients cisgender whose subjective history is crossed by a positive laboratory result. The interest in gender dynamics as an important analytical operator when the subject is HIV/AIDS – as highlighted by several studies on the subject - and the primacy of the relationship with others as a constitutive element for the human subject, resulted in the election of a study focused on to the metapsychological universe of the Self in its internal relations with narcissism and the superego, and with its sociopolitical context. To this end, clinical illustrations were used with material from the researcher's care memory in his work with patients in hospital on the context of discovering their serology. The effort to reposition psychological suffering as a methodological guide for conducting research is highlighted. Finally, this research seeks to deepen the psychoanalytic understanding of the representational and identification mechanisms that participate in egoic functioning and its investment in maintaining na identity. It does so by mapping the effects of trauma in the clinic with men who discover they are positive for HIV/AIDS within the scope of tertiary health care, with the aim of improving theoretical-technical tools for psychotherapeutic work with this public.
